摘 要:目的研究增龄对腹部创伤合并感染脓毒血症大鼠的急性胰岛素抵抗的影响。方法实验大鼠按周龄分3组:4周龄组、8周龄组、12周龄组。行盲肠穿孔术(CLP术),同时腹腔内注射内毒素,模拟创伤脓毒血症导致高血糖的动物模型,测定其鼠尾毛细血管血糖。观测各组创伤应激后不同时间的血糖、血清胰岛素水平,行经腹腔注射葡萄糖耐量试验和高胰岛素正葡萄糖钳夹试验。蛋白质印迹法(Western blot)检测大鼠肝组织Psr473AKt/AKt、P-mTOR/mTOR的表达比率。结果随着周龄增加,创伤后血清胰岛素水平明显升高,高血糖和低血糖的发生率也增加。增龄对大鼠经腹腔注射葡萄糖耐量和高胰岛素正葡萄糖钳夹试验结果具有显著影响。增龄减低大鼠肝组织Psr473AKt/AKt和P-mTOR/mTOR的表达比率。结论增龄可能通过影响肝组织胰岛素信号通路AKt和mTOR的磷酸化水平,从而加剧创伤感染应激后大鼠急性胰岛素抵抗程度。Objective To investigate the effects of aging on surgery trauma inducing acute insulin resistance in Wistar rats.Methods We performed surgical abdominal trauma in male rats of three different ages(4-,8-,and 12-weeks old). Rats were accepted abdominal surgery addition to injection of endotoxin by 3 mg/kg. Peripheral blood sugar,serum insulin, glucose level of intraperitoneal injection for glucose tolerance and glucose infusion rate by hyperinsulinemic euglycemic clamp were detected.Expression of p-AKt/AKt and p-mTOR/mTOR in liver were measured by western blot. Results Serum insulin levels were obviously increased after abdominal surgery trauma. Glucose level of intraperitoneal injection for glucose tolerance was increased in the group of12 weeks age. Glucose infusion rate was increased in groups of elder age. The ratio of p-AKt/AKt and p-mTOR/mTOR was down regulated as age increases. Conclusion These results indicate that aging increase trauma-induced acute insulin resistance affecting phosphorylation of AKt and mTOR insulin signaling pathway in liver, hence attenuate insulin resistance following trauma infection stress.
