消痔灵注射液治疗Ⅲ度直肠脱垂的疗效及血清铝水平的变化

摘　　要：目的:观察不同剂量消痔灵注射治疗Ⅲ度直肠脱垂的疗效、安全性及对血清铝离子水平的影响。方法:将90例本病患者随机分为低、中、高剂量组,采用直肠黏膜下注射和直肠周围注射,检测不同时相血清铝离子浓度。结果:低剂量组总有效率为70.0%;中剂量组总有效率为90.0%;高剂量组总有效率为90.3%,3组比较,高剂量组疗效优于中、低剂量组,差异有统计学意义(P<0.05)。其中3组好转的21例中有18例采用再次注射而痊愈,另3例行PPH术治愈;无效的14例中有9例通过再次注射治愈,另5例通过PPH手术而治愈。给药剂量越大,患者给药后血清中铝离子浓度越高;给药12 h后血清铝离子浓度达最高峰为607.1μg/L,7 d后血清铝离子浓度均基本达到正常水平。结论:采用高剂量消痔灵注射液注射治疗Ⅲ度直肠脱垂可取得更好的疗效;消痔灵注射治疗Ⅲ度直肠脱垂安全、有效。Objective： To evaluate the therapeutic effect, safety and dynamic influence on serum aluminum level with different doses of Xiaozhiling Injecting at third degree rectal prolapse. Methods： The 90 patients were randomly divided into low dose group, middle dose group and high dose group, and respectively received under-rectal mucosa or per/rectal inject therapy. The serum aluminum level was tested in different phases after treatment. Results： The general effective rate was 70.0% in low dose group, 90.0% in middle dose group, and 90.3% in high dose group. The efficacy of the high dose group was better than the other two group（P〈0.05）. Among the 21 improved patients, 18 patients were healed after the second time injection and the other 3 were taken PPH. Among the 14 cases which were inefficiency, 9 were healed after the second time injection and the other 5 were taken PPH. The bigger the dose, the higher the concentration of serum aluminum. The serum aluminum ion concentration reached peak 12 hours later after administration, and 7 days later, the serum aluminum ion concentration reached the normal level. Conclusion： High dose of Xiaozhiling Injecting can get better efficacy on Third Degree Rectal Prolapse. And the therapy is safe, effective.
