高血压病中医诊疗指南真实世界适用性分析

摘　　要：目的:分析高血压病中医诊疗指南临床使用情况,评价其临床适用性,为指南的修订提供依据。方法:回顾性调查800例高血压住院患者,采集病历号、年龄、性别、病程、初发症状、临床表现、西医诊断、中医诊断和处方等信息,比较、统计和分析指南推荐证候和处方与临床实际的一致性。结果:52.03%的病例按照指南推荐建议进行了处方,大部分医生在临床实践中参考了高血压病中医诊疗指南的推荐建议。指南给出的证候占临床常见证候的58.38%,其中无诊断为冲任失调证的病例,而临床病例中出现较多的痰瘀互结证指南中未推荐。处方分析表明除治疗肾精不足的常用处方与推荐建议不一致外,其他临床应用处方基本与指南推荐一致。结论:高血压病中医临床诊疗指南存在部分不适用之处,建议进一步修订。Objective： To evaluate the clinical applicability and provide the proof for revising the guideline by analyzing the use of TCM diagnosis and treatment guideline for hypertension in the real world. Method： Retrospective survey 800 cases hypertension in-patients. Record number, age, gender, course of disease, syndrome, western diagnosis, Chinese diagnosis and prescription were recorded. The consistency between syndrome and prescription recommended by guideline and clinical practice was analyzed. Results： 52.03% cases were prescribed according to recommendation provided by guideline, which illustrated that most doctors used the recommendation provided by guideline during clinical practice. Syndrome provided by guideline accounted for 58.38% of clinical common syndrome. The case diagnosed as imbalance of Chong and Ren channel was none in clinical. The syndrome of combination of phlegm and blood stasis which was more frequent in clinical was not recommended by guideline. Analysis of prescription indicated that prescription was agreed with guideline but prescription aiming to kidney essence deficiency. Conclusion： Part recommendation was not applied and TCM diagnosis and treatment guideline for hypertension should be revised.
