摘  要:目的:从抗氧化酶活力、脂质过氧化、蛋白质过氧化和细胞核过氧化角度观察清化颗粒对"热"证2型糖尿病患者氧化应激水平的影响。方法:在上海中医药大学附属曙光医院内分泌科招募符合中医"热"证素特征的2型糖尿病患者60例,在固定原用药方案基础上,随机分组,治疗组加用清化颗粒,对照组加用安慰剂,每日1袋,冲服,疗程为12周。服药前后分别于空腹、标准馒头餐后120min留取血清。主要评价指标:丙二醛、8-异前列腺素F2α、晚期氧化蛋白产物、8-羟基脱氧鸟苷、超氧化物歧化酶、谷胱甘肽过氧化物酶。次要评价指标:空腹血糖、餐后血糖、糖化血红蛋白、血脂。结果:治疗组超氧化物歧化酶上升,丙二醛下降,晚期氧化蛋白产物下降,组间存在差异(P<0.05)。治疗组糖化血红蛋白下降(P<0.05),空腹血糖、餐后血糖、血脂等指标未见改变。结论:清化颗粒能提高抗氧化酶活力,减轻脂质和蛋白质过氧化损伤,改善糖尿病患者的氧化应激程度。Objective: To evaluate the influence of Qinghua Granule (QHG) on oxidative stress level of type-2 diabetics with heat syndrome from antioxidant enzyme activity, lipid, protein and cell nucleus peroxide. Methods: 60 type-2 diabetics with heat syndrome were recruited and randomly divided into control group and treatment group. On the basis of fixed treatment, patients in treatment group were treated with QHG, and the patients in control group were treated with placebo lasted for 12 weeks. Blood serum was collected on an empty stomach and 120 minutes after the standard steamed bread meal. The main evaluation indexes included malondialdehyde (MDA), 8-prostaglandin F2tt, advanced oxidation protein products (AOPP), 8-hydroxy deoxyguanosine, superoxide dismutase (SOD) and glutathione peroxidase (GPX). The secondary evaluation indexes included fasting blood-glucose (FBG), postprandial blood glucose (PBG), glycosylated hemoglobin and blood fat. Results: Superoxide dismutase (SOD) of patients in treatment group increased. MDA and AOPP of patients in treatment group decreased. There were significant differences between the two groups (P〈0.05). Glycosylated hemoglobin of patients in treatment group was decreased. FBG, PBG and blood fat had no change. Conclusion: QHG could improve the antioxidant enzyme activity, relieve peroxidation damage of lipid and protein and improve the level of oxidative stress of diabetic patients.
