尿蛋白指标联合检测在高血压肾病早期诊断中的价值

摘　　要：目的探讨不同时期的高血压患者尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、β2微球蛋白(β2-MG),尿微量白蛋白(mALb)含量的改变对肾脏早期损伤的诊断价值。方法选取2012年1月—2013年10月就诊于本院的高血压患者200例为高血压组,测定其β2-MG、NAG、mALb、尿肌酐等指标。另设立健康对照组57例。结果高血压组尿NAG、β2-MG、mALb水平均高于健康对照组,差异有统计学意义(P<0.05),NAG+β2-MG+mALb三项联合检测的阳性率高于二项联合检测的阳性率以及单项检测的阳性率,差异有统计学意义(P<0.05)。结论尿NAG、β2-MG和mALb是诊断肾脏早期损伤的灵敏指标,联合检测可显著提高阳性检出率,对及时准确地诊断高血压早期肾损害有非常重要的临床应用价值。Objective To discuss the diagnostic value of urinary N - acetyl - β - D - Glucosaminidase （ NAG）, be- ta -2 microalbumin （ β2 - MG ）, microalbuminuria （mALb） changing for early renal injury in different periods of patients with hypertension. Methods A total of 200 patients with hypertension in our hospital from January 2012 to October 2013 were chosen as hypertension group, β2 - MG, NAG, mALb, urine creatinine were detected. And other healthy 57 cases were chosen as control group. Results The levels of urine NAG, β2 - MG, mALb in hypertension group were higher than those of control group, the differences were statistically significant （ P 〈 0.05 ） ; the positive rate of NAG ＋ β2 - MG ＋ mALb combined detection was higher than both or single detection, the differences were statistically significant （ P 〈 0. 05 ）. Conclusion Urine NAG, β2 - MG and mALb are sensitive indicators for diagnosis of early renal injury in patients with hypertension, combined detection can significantly increase the positive rate, which has very important clinical value in timely and accurate diagnosis of early renal injury.
