摘 要:目的:探讨并得出吉林省健康儿童经体表面积(BSA)调整的冠状动脉的Z值,运用Z值来判断冠状动脉是否异常,从而指导可能影响冠状动脉的疾病的临床诊治.方法:研究收录了2009-05~2012-10吉林大学第一医院儿科超声门诊检查的15岁以下健康儿童,共998例.将其分成6组,分别为≤1岁、1~3岁、3~6岁、6~9岁、9~12岁、12~15岁.使用东芝SSH-880CV仪器分别测量左冠状动脉主干(LMCA)和右冠状动脉(RCA)内径.将测量的内径值转换为经BSA调整的Z值.结果:在不同年龄组的健康儿童之间经BSA调整的LMCA和RCA内径Z值存在明显差异.Z值在低年龄组之间以及高年龄组之间没有统计学意义.结论:该研究建立了吉林健康儿童冠状动脉Z值的正常区间,运用Z值来判断冠状动脉是否扩张,对于指导川崎病以及其他可能影响冠状动脉的疾病的临床诊治工作有着重要的意义.Objective: To find Body Surface Area (BSA) adjusted Z--Score of coronary artery in healthy children and to judge the coronary artery (CA) abnormality based on BSA adjusted Z--Score. The usage of Z score to detect coronary artery dilatation can serve as a better index in coronary artery involved diseases which can have a direct impact on the clinical treatment of the disease. Methods: This study was conducted from May 2009 to December 2012. This study included a total of 998 healthy children under 15 years old who came to the Pediatric echocardiography department of the First Hospital of Jilin University. They were categorized into six groups based on their ages, ≤1 year old group, l year old group, 3 years old group, 6 years old group, 9 years old group, 12~15years old group. We measured the diameters of left main coronary artery (LMCA) and the right coronary artery (RCA) by TOSHIBA SSH--880CV. The measured dimensions were converted to BSA adjusted Z Scores. Results: There were significant differences in BSA adjusted Z--Scores of LMCA and RCA in different age groups of healthy children. There was no statistical significance in Z score amongst young age groups as well as older age groups. Conclusion: This study establishes a correct and reliable method to calculate coronary artery Z score in Chinese population. The usage of Z score to detect coronary artery dilatation can serve as a better index in Kawasaki disease and other coronary artery involved diseases, which can have a direct impact on the clinical treatment of the disease.
