神经节苷脂联合前列地尔治疗糖尿病周围神经病变的疗效观察

摘　　要：目的 探讨前列地尔联合单唾液酸四己糖神经节苷脂（GM-1）治疗糖尿病周围神经病变（DPN）的临床疗效.方法 选取2013年2月～2014年2月收治的DPN患者80例,按照随机数字表法分为观察组和对照组,每组40例.对照组给予GM-1治疗,观察组在对照组的基础上加用前列地尔治疗,两组均以14 d为一个疗程.观察对比两组治疗前后临床症状体征和肌电图检查（正中神经和腓总神经的运动传导速度MNCV和感觉传导速度SNCV）的变化,评价并对比两组的临床疗效.结果 观察组和对照组的总有效率分别为95.0％ vs 67.5％,观察组明显优于对照组,差异有统计学意义（x2=9.9282,P＜0.01）;治疗前两组患者的正中神经和腓总神经的MNCV和SNCV比较差异无统计学意义（P＞0.05）;治疗后两组患者的正中神经和腓总神经的MNCV和SNCV均较治疗前明显改善,观察组改善情况明显优于对照组,差异有统计学意义（P＜0.01）;两组患者均未发生严重不良反应.结论 前列地尔联合单唾液酸四己糖神经节苷脂治疗糖尿病周围神经病变临床疗效显著,可显著改善患者的症状体征和神经传导速度,值得临床推广应用.Objective To investigate the clinical curative effect of ganglioside （GM-1） combined with alprostadil in the treatment of diabetic peripheral neuropathy（DPN） .Methods 80 cases with DPN admitted to hospital from February, 2013 to February,2013 ,were divided into observation group and control group with 40 cases in each group according to random number table.The control group were treated with GM-1, based on which the observation group were added with alprostadil treatment.Both groups were treated for 14 days （one course）.Changes of clinical symptoms and signs ,of electromyography ex- amination for motor nerve conductive velocity（MNCV） and sensory nerve conduction velocity （SNCV） of median nerve and common peroneal nerve before and after treatment were observed. Meanwhile, clinical effects of the two groups were evaluated and compared.Results The total effective rates of the observation group and the control group were 95.0% and 67.5% re- spectively, and that of the observation group was significantly higher than that of the control group, so the difference was sta- tistically significant（ X2= 9.9282, P〈O. O1 ）.The MNCV and SNCV of the median nerve and common pemneal nerve of two groups were not statistically different before treatment（P〉O. 05）, but they both significantly improved after treatment. Those of the observation group improved more significantly, so the difference was statistically significant（ P〈0. 01 ）. No serious ad- verse reactions were found in two groups. Conclusion Ganglioside combined with alprostadil has positive clinical effect in the treatment of diabetic peripheral neuropathy, because it can obviously relieve patients＇ clinical symptoms and signs and nerve conduction velocity. Thus, it is worthy of clinical popularization and application.
