伽玛刀放射外科治疗原发性三叉神经痛的长期疗效及相关因素分析

摘　　要：目的:研究伽玛刀放射外科治疗原发性三叉神经痛的长期有效性和安全性。方法:对随访时间超过60个月、仅接受过1次伽玛刀放射外科治疗的原发性三叉神经痛患者进行回顾性分析,总结患者的长期疼痛控制率、并发症发生率并进行相关因素分析。结果:对69例患者的平均随访时间为92个月。疼痛控制率I级14例(占20.3%);II级10例(占14.5%);III级26例(占37.7%);IV级13例(占18.8%);V级6例(占8.7%)。11例(占15.9%)患者出现面部感觉异常,无严重不良反应。患者的年龄、性别及疼痛的部位不影响伽玛刀的疗效。结论:伽玛刀放射外科治疗三叉神经痛疗效确切,不良反应发生率低,是不能耐受手术患者的较好治疗方式。Objective： To assess the long-term safety and efficacy of gamma knife radiosurgery for treatment of typical trigeminal neuralgia （TN）. Methods： Retrospectively analyzed the typical trigeminal neuralgia patients who received only single gamma knife radiosurgery treatment and the follow-up time for more than 60 months, To assess the long-term safety and efficacy of the treatment for TN, the related factors were analyzed at last. Results： A total of 69 patients were enrolled in this study and the mean follow-up time were 92 months（60-128 months）. According to the pain intensity scale at the last follow-up, 14 cases（20.3%） were grade I, 10 cases（14.5%） were grade II, 26 cases（57.7%） were grade III, 15 cases（18.8% ） were grade IV and 6 cases（8.7%） were grade V. Facial sensory dysfunction occurred in 15.9% of patients, but no serious treatment- related complications were found. The age, sex and pain site does not affect the gamma knife＇s efficacy. Conclusion： Gamma knife radiosurgery is a safe and effective long-term treatment method for TN and should be considered as an alternative option for the patients who cannot tolerate invasive surgery.
