丙种球蛋白治疗小儿病毒性心肌炎的效果及其对患儿免疫功能的改善作用

摘　　要：目的探讨丙种球蛋白辅助治疗小儿病毒性心肌炎的效果及其对患儿免疫功能的改善作用。方法选择本院2012年1月-2014年3月收治的小儿病毒性心肌炎患者76例,随机分为观察组和对照组,各38例,对照组患儿给予常规治疗,观察组患儿在常规治疗的基础上给予丙种球蛋白静脉滴注治疗,治疗4周后评价两组的治疗效果,治疗前后测定患儿T细胞亚群（CD3＋、CD4＋、CD8＋）变化情况。结果对照组治疗总有效率为76.32%,观察组治疗总有效率为92.10%,两组差异有统计学意义（P〈0.05）。两组治疗前CD3＋、CD4＋、CD8＋、CD4＋/CD8＋水平差异无统计学意义（P〉0.05）,治疗后观察组CD3＋、CD4＋、CD8＋明显高于对照组,CD4＋/CD8＋明显低于对照组,差异有统计学意义（P〈0.05）。结论丙种球蛋白辅助治疗小儿病毒性心肌炎可提高治疗效果,并且能有效改善患儿的免疫功能,值得临床推广应用。Objective To evaluate the effect of gamma globulin in treatment of children with viral myocarditis and its improvement role on immune function. Methods 76 cases of children with viral myocarditis from January 2012 to March 2014 in our hospital were selected and randomly divided into observation group and control group,each of 38 cases,the children in the control group were given with conventional treatment,patients in the observation group were given with intravenous gamma globulin treatment on the basis of conventional therapy.The effects of two groups were e-valuated after 4 weeks of treatment.Before and after treatment,T cell subsets （CD3＋,CD4＋,CD8＋） changes of two groups were measured. Results The total effective rate of control group and observation group was 76.32% and 92.10% respec-tively,the difference was significant （P〈0.05）.Before treatment,CD3＋,CD4＋,CD8＋,CD4＋/CD8＋level of two groups showed no significant difference （P〉0.05）,after treatment,the CD3＋,CD4＋,CD8＋level of observation group was higher than that of con-trol group,CD4＋/CD8＋ level was lower than that of control group,the differences were significant （P〈0.05）. Conclusion Gamma globulin adjunctive therapy in children with viral myocarditis can effectively improve the therapeutic effect,and can effectively improve immune function of children,worthy of widely application.
