摘　　要：目的:运用二维超声心动图(2DE)、Tei指数、定量组织速度成像(QTVI)评价矽肺患者右室功能的改变。方法:对照组30例,矽肺组30例,所有研究对象行2DE右心常规超声指标测量;脉冲组织多普勒(PWTDI)模式下测算右室Tei指数;QTVI获取三尖瓣前叶及隔叶瓣环的心肌运动速度曲线,记录收缩期峰值运动速度(VS)、舒张早期运动速度(VE)、舒张晚期运动速度(VA),计算VE/VA。结果:与对照组相比,矽肺组右心大小及肺动脉压无明显变化,右室Tei指数明显增大(P<0.01);心肌VA增加、VE降低,VE/VA值降低(P<0.05或P<0.01)。结论:右室Tei指数及QTVI测量的右心室局部心肌组织运动速度可以早期、敏感、准确地反映矽肺患者右室功能的变化,是评价矽肺患者右室功能的较好指标。Objective： To evaluate right ventricular function in patients with silicosis using two-dimensional echocardiography（ 2DE）,Tei index and quantitative tissue velocity imaging（ QTVI）. Methods： Thirty patients with silicosis and 30 normal controls were enrolled in the study. Traditional parameters of right heart diameter were studided by 2DE,right ventricular Tei index were studided by TVI. QTVI parameters were measured from the tricuspid annular systolic and diastolic velocity. The following parameters were analyzed： the ventricular myocardial systolic velocity（ VS）,early diastolic velocity（ VE）,late diastolic velocity（ VA）,VE/VAratio. Results： Compared with the control group,Tei index were significantly increased in silicosis group（ P 〈 0. 01）. Some parameters such as VAwere increased,VEand VE/VAratio were reduced（ P 〈 0. 05 or P 〈 0. 01）. Conclusion： Tei index and QTVI may provide a quantitative,early,simple and noninvasive useful tool for right ventricular function evaluation.
