摘 要:目的研究CD40及CD154(CD40L)在不同时间段糖尿病肾损害大鼠外周血单个核细胞和肾小管上皮细胞的表达及作用。方法雄性SD大鼠40只,随机分为对照组10只和实验组30只。实验组用链脲佐菌素(STZ)60mg/kg一次性腹腔注射SD大鼠,分别于1、4、12周将模型组与对照组大鼠处死,流式细胞术测定外周血单个核细胞表面CD40及CD154的表达;取肾脏组织,做病理切片及抗CD40免疫组织化学染色。结果 HE染色显示,与正常对照组相比,模型组有炎症细胞浸润,肾小管上皮细胞损伤和肾小球膨胀,基底膜增厚。注射链脲佐菌素1周后与对照组相比,CD40-CD154+在单个核细胞中的表达无差异。4、12周与对照组相比,CD40-CD154+在单个核细胞中的表达上升(P<0.05);免疫组化染色显示,4周和12周肾实质细胞CD40表达高于对照组(P<0.05)。结论在模型组外周血单个核细胞表面CD154的表达升高和肾小管上皮细胞CD40的表达上调在糖尿病大鼠的肾损伤中起了非常重要的作用。Objective The study analyzed the expression and function of CD40 and CD154 (CD40L) on peripheral blood mononuclear cells (PBMCs) and renal tubular epithelial cells of diabetic rats with kidney injury at different time points. Methods 40 male SD rats were randomly divided into a control group (10 rats) and an experimental group (30 rats). The experimental group was intraperitoneally injected with 60 mg/kg of streptozotocin once, and the model group and the control group were sacrificed at the 1st, 4th, and 12th weeks. Flow cytometry was used to measure the expression of CD40 and CD154 on the surface of PBMCs. Kidney tissues of the rats were removed and sectioned for anti-CD40 immunohistochemical staining. Results HE staining results showed that as compared with the normal controls, the model group had inflammatory cell infiltration, renal tubular epithelial cell damage, and glomerulus expansion, and thickening of the basal membrane. One week after the injection with streptozotocin, no difference in the expression of CD40-CD154~ on PBMCs was observed between the model group and the control group. At the 4~ and 12~ weeks, the expression of CD40-CD154+ on PMBCs of the model group increased as compared with the control group (P〈0.05). Immunohistochemical staining showed that CD40 expression in renal parenehyma ceils of the model group was higher than the control group at the 4~ and 12~ weeks (P〈0.05). Conclusion The upregulation of CD154 expression on PBMC surface and CD40 expression in renal tubular epithelial cells has shown to play a critical role in renal injury of diabetic rats.
