摘　　要：目的探讨一期翻修术治疗全膝关节置换术后感染的护理干预方案。方法选择2005年4月-2012年12月在新疆医科大学第一附属医院经一期翻修术治疗全膝关节置换术后感染54例患者,制订适合膝关节翻修手术的护理干预方案,特别是患者的病情观察、术区护理、引流管护理、感染控制、合理使用抗生素、个性化的功能锻炼等方面。结果 54例行一期膝关节翻修手术的患者,经过手术治疗及护理干预后,52例伤口均一期愈合,术后2w出院时均可扶拐行走,无明显疼痛和关节不稳,住院期间无跌倒、坠床、压疮、下肢深静脉血栓等并发症,患肢功能均恢复良好。2例功能欠佳,KSS功能评分平均50分,其余52例至末次随访时KSS功能评分平均94分。患者满意度达到了92.6%。结论人工膝关节置换术后感染行一期翻修手术的护理干预方案有效、可行,可促使患者患肢功能得到最大限度地恢复。患者对围手术期的治疗、护理、康复效果均感到满意。Objective Discuss the nursing intervention programs in the first-stage revision surgery for total knee arthroplasty postoperative infection.Methods We collected 54 cases with infection after total knee arthroplasty from April 2005 to December 2012.The patients received the first-stage revision surgery to treat the postoperative infection.And then we worked out suitable nursing intervention programs for them. The nursing programs focused on disease observation,nursing on operation area and drainage tube,infec-tion control,and personalized functional exercise.Results Among 54 patients who received the first-stage revision surgery and nursing intervention treatment perioperatively,52 cases recovered completely and were discharged from hospitals after 2 weeks.But 2 cases could not function well after the treatment,and the KSS score is 50 by average.The KSS score for the other 52 cases reached 94 on average.The patients′satisfaction with the treatment is up to 92.6%.Conclusion The nursing intervention programs in the first-stage revision surgery for total knee arthroplasty postoperative infection are effective and feasible and helpful in patients＆#39;maximum recovery.Patients are very satisfied with the result of treatment,nursing and recovery.
