钬激光碎石辅助后腹腔镜治疗鹿角形结石的疗效观察

摘　　要：[目的]探讨钬激光碎石辅助后腹腔镜治疗鹿角形结石的疗效。[方法]入选的118例病例均为在我院诊治并被确诊的鹿角形结石患者,其中2012年9月至2013年12月采用钬激光碎石辅助后腹腔镜治疗鹿角形结石29例患者为治疗组;2008年9月至2012年9月采用单纯体外震波碎石(extracorporeal shock wave lithotripsy,ESWL)28例患者为对照1组,采用经皮肾镜取石(percutaneous nephrolithotomy,PCNL)29例患者为对照2组,采用腹腔镜手术32例患者为对照3组。观察比较组间的治疗周期,结石取净率,以及肾脏损伤、肾盂撕脱、术后尿漏等并发症(percutancous nephrolithotomy,PCNL)。[结果]治疗周期、结石取净率和肾脏损伤、肾盂撕脱、术后尿漏等并发症比较,治疗组均优于所有对照组(P<0.05)。[结论]钬激光碎石辅助后腹腔镜治疗鹿角形结石的疗效明确,值得临床运用并进一步探讨。[Objective] To discuss the cure effect of antler-shape stone treated with retroperitoneoscopy coordinated with holmium laser macadam.[Method]Select 118 cases diagnosed as ander-shape stone,among which,29 cases were in treatment group taking holmium laser macadam coordinated for retroperitoneoscopy; other 28 cases in control group Ⅰ,only taking pure seismic wave macadam in vivo,and another 29 cases taking percutaneous nephroscope calculus removal as control group 2; another 32 cases taking laparoscope as control group 3.Observe and compare their treatment course,calculus removal rate,kidney injury,pelvis avulsion,post-operation urine leak and other complications.[Result] On comparison of treatment circle,stone removal rate,kidney injury,pelvis avulsion and post-operation urine leak,the treatment group was better than all control groups.[Conclusion] Holmium laser coordinated in retroperitoneoscopy treating ander-shape calculus has definite cure effect,worth clinical application and further discussion.
