浙江省三级甲等医院无创正压通气使用现状调查

摘　　要：目的了解无创正压通气在浙江省三级甲等医院的使用现状,以便提升无创正压通气临床使用率和有效性。方法采用问卷调查法,调查内容包括调查对象的基本情况、护士对使用无创正压通气技术的认知和无创正压通气治疗临床使用现状。通过在相关会议上发放调查表,当场回收调查表,并选取三级甲等医院呼吸科或重症监护室护士的问卷,将问卷数据输入统计软件进行统计分析。结果调查回收纳入研究的问卷共241份,来自浙江省16家三级甲等医院。结果显示COPD、睡眠呼吸暂停综合征和急性心源性肺水肿是无创正压通气治疗使用最主要的适应证。90.04%调查对象对无创正压通气治疗表现出积极的态度,47.72%的护士认为自己的技术很成熟和成熟,经常使用无创正压通气治疗的护士比例仅占48.54%。63.07%护士认为本科室未充分使用无创正压通气治疗,其原因依次是操作者未得到充分训练、人员配备不足、患者依从性差。结论浙江省三级甲等医院护士对无创正压通气治疗持积极肯定的态度,但实际使用率偏低。Objective To study the using actuality of noninvasive positive pressure ventilation in Grade A Class Three hospitals of Zhejiang and improve clinical using rate and effectiveness of noninvasive positive pressure ventilation. Method Investigate on general information, nurses＇ cognition on using skills of noninvasive positive pressure ventilation and clinical using actuality. Questionnaires are sent out and received on meeting. Choose questionnaires from nurses of respiratory de- partment or ICU of Grade A Class Three hospitals. Put data into software and make analysis. Result 241 questionnaires from 16 Grade A Class Three hospitals of Zhejiang are used for the research. Result shows that COPD, sleep apnea syn- drome and acute cardiogenic pulmonary edema are the main indications of noninvasive positive pressure ventilation. 90. 04% patients show positive attitude towards noninvasive positive pressure ventilation. 47. 72% nurses are confident in their skills. Only 48. 54% nurses often use noninvasive positive pressure ventilation. 63. 07% nurses think their department doesn＇t make full use of noninvasive positive pressure ventilation. The reasons are inadequate training, staffing and poor patients＇ compliance. Conclusion Nurses from Grade A Class Three hospitals of Zhejiang have positive attitude towards non- invasive positive pressure ventilation, but the using rate is low.
