老年早期慢性肾衰竭患者肾功能恶化的临床危险因素研究

摘　　要：目的进一步研究和探讨导致老年早期慢性肾衰竭患者肾功能恶化的危险因素,为临床的相关研究提供依据。方法选取2012年1月至2013年6月菏泽市立医院收治的164例早期慢性肾衰竭,并且出现肾功能恶化的患者为研究对象,按照患者的年龄分成≥60岁组和<60岁组,针对导致肾功能恶化发生的临床危险因素对两组患者进行分析和比较研究。结果≥60岁组患者中因严重感染、肾毒性药物和低血容量等因素导致肾功能恶化发生的比例显著高于<60岁组患者的发生比例,组间比较差异有统计学意义(P<0.05);<60岁组因原发病复发进展和恶性高血压因素导致肾功能恶化发生的比例显著高于≥60岁组的发生比例,组间比较差异有统计学意义(P<0.05);多因素Logistic回归分析,结果显示严重感染(OR=9.432)、肾毒性药物(OR=8.421)、低血容量(OR=6.653)是老年早期慢性肾衰竭患者肾功能恶化的独立危险因素。结论严重感染、肾毒性药物和低血容量等因素是导致老年早期慢性肾衰竭患者肾功能恶化的临床危险因素。Objective To explore the clinical risk factors that could lead to renal function deterioration in elderly patients with early chronic renal failure,and provide reference for clinical research.Methods A total of 164 cases of patients with early chronic renal failure and renal function deterioration admitted in Heze Municipal Hospital from Jan.2012 to Jun.2013 were divided into ≥60 years old group（elder group） and ＜ 60 years old group（young group） according to their ages,clinical risk factorrs that could lead to renal function deterioration of the two groups were analyzed and compared.Results In the elder group,proportion of patients with renal function deterioration caused by the factors of serious infections,drug renal toxicity and low blood volume was higher than that in the young group,there was statistically significant difference between the two groups （P ＜ 0.05） ; in the young group,proportion of patients with renal function deterioration caused by the factors of recurrence and progression of the original disease and malignant hypertension was significantly higher than that of the elder group,there was statistically significant difference between the two groups（P ＜ 0.05） ; result of multivariate logistic regression analysis showed that severe infection （OR =9.432）,drug renal toxicity （OR=8.421）,low blood volume （OR=6.653） were independent risk factors of renal function deterioration in elderly patients with early chronic renal failure.Conclusion Severe infection,drug renal toxicity and low blood volume are the clinical risk factors of renal function deterioration in elderly patients with early chronic renal failure.
