氯吡格雷联合瑞舒伐他汀钙及优质护理对短暂性脑缺血的疗效评价

摘　　要：目的探讨氯吡格雷联合瑞舒伐他汀钙对短暂性脑缺血(TIA)的临床疗效,以及其对患者生存质量的改善效果。方法按入院顺序将95例TIA患者随机分成两组。对照组48例患者常规给予阿司匹林肠溶片100 mg/d;治疗组47例患者在此基础上给予硫酸氢氯吡格雷片75 mg/d,瑞舒伐他汀钙片10 mg/d(睡前服用)。治疗第7个月时实施优质护理行动,观察其对TIA患者的影响。结果通过14个月的治疗,对照组总有效率为62.50%,发作频率(3.2±1.5)次/月;治疗组总有效率为80.85%,发作频率(1.8±0.2)次/月;治疗后,治疗组血小板计数(PLT)明显降低,凝血活酶时间(APTT)、凝血酶时间(PT)较治疗前延长(P<0.05),对照组PLT,APTT,PT与治疗前比较差异无统计学意义;采用有针对性的优质护理服务后,患者对护理的满意度大大提高,医疗纠纷发生率降低。结论氯吡格雷联合瑞舒伐他汀钙大大提高了短暂性脑缺血的疗效,优质护理服务有利于降低医疗纠纷。Objective To investigate the clinical effect of clopidogrel combined with rosuvastatin calcium in treating transient cerebral is-chemia（TIA）,and to explore the improvement effect of quality of life in the patients by conducting simultaneous high quality nursing service. Methods 95 patients with TIA were randomly divided into two groups according to the sequence number of admission. The control group（48 cases）was treated with the conventional medicine Aspirin Enteric-Coated Tablets 100 mg/d;on this basis the treat-ment group（47 cases）was given Clopidogrel Hydrogen Sulfate Tablet 75 mg/d,Rosuvastatin Calcium Tablet 10 mg/d（before sleep）. In 7 months during the treatment,the high quality nursing service was implemented in our hospital. Thus the effect of the high quality nursing service on the TIA patients was observed. Results The total effective rate after 14-month treatment in the control group was 62.50%,the seizure frequency was（3.2±1.5）times/month,which in the treatment group were 80.85% and（1.8±0.2）times/month respectively;PLT after treatment in the treatment group was significantly reduced（ P 〈 0. 05）,APTT and PT were prolonged compared with before treatment;while PLT,APTT and PT in the control group had no statistical differences between before and after treatment;af-ter the adopting targeted high quality care service,the patient satisfaction on the nursing care was greatly increased and the occurrence rate of medical disputes was significantly decreased. Conclusion Clopidogrel combined with rosuvastatin calcium enormously increases the effect in treating treatment TIA,the high quality nursing service conduces to reduces the medical disputes.
