摘　　要：目的探讨人乳头瘤状病毒(Human Papillomavirus,HPV)18型感染及其整合状况在宫颈病变细胞中的意义。方法应用多聚酶链反应(Polymerase Chain Reaction,PCR)及荧光定量PCR方法,对HPV18 E6、E2及人管家基因β-actin的拷贝数进行检测,分析HPV18整合状况在宫颈病变中的意义。结果 428例标本中9例HPV18阳性,其中1例为纯游离型,即E2与E6比率大于1;3例为纯整合型,即E2/E6比率等于零;5例为整合与游离的混合型,即E2/E6比率在0与1之间;并且随游离型比例降低,整合型(游离+整合)比例升高,病变程度加重,检测到3例整合型标本定位于患者的8号染色体。结论 HPV18整合入宿主基因组可能是宫颈癌变的关键因素;提示,通过HPV18整合状况的早期筛查,可以及早发现宫颈癌变的高危患者。Objective To explore the significance of human papilloma virus （HPV） 18 infection and its integration in cervical lesions cells. Methods HPV18 was detected by polymerase chain reaction （PCR）. The copies of HPV18 E2, E6 and human housekeeping gene 13 -actin were tested by quantitative PCR for analysis on the relationship between HPV18 integration and cervical lesions. Results Total 9 cases were HPV18-positive from 428 specimens. Among the samples, one case was pure episomal with E2/E6 ratios 〉 1; Three cases were pure integrated with E2/E6 ratio= zero; Five cases were mixture of integration and episomal with 0〈E2/E6 ratios〉 1. The prevalence of episomal type decreased gradually and the prevalence of integrated （episomal and integration） forms increased with progression of cervical disease, and 3 cases integrated into chromosome 8. Conclusion HPV18 integrated into the host genome that may be a key factor in cervical lesions, that suggests screening HPV18 integration early, the patients of high risk for cervical lesions can be found by screening HPV18 integration early.
