小茴香热熨用于肝硬化门奇断流术后胃肠功能恢复的疗效探讨

摘　　要：目的 探讨小茴香热熨用于肝硬化门奇断流术后胃肠功能恢复的的疗效。方法 将59例肝硬化门奇断流术病人分成治疗组、对照组,治疗组术前、术后常规护理（术前：卧床,合理饮食,对症保肝、止血、利尿、营养支持,注意处理并发症而且对患者健康教育;术后：卧床,注意姿势,从流食到普食过度,密切观察病情及时反馈与记录,预防并发症并对患者进行健康教育）加上术后用中药小茴香行腹部热熨治疗,于神阙穴覆盖消毒纱布,将药熨袋于神阙穴按压,并以此中心顺时针揉腹;对照组术前、术后常规护理（方法同治疗组）加上术后用TDP照射腹部治疗术后12 h用TDP照射腹部,每次15~20 min,2次/d。结果 治疗组术后肠鸣音恢复时间（48.33±13.68）、第1次肛门排气时间（70.76±15.32）与对照组相比肠鸣音恢复时间（60.27±13.32）,第2次肛门排气时间（82.37±15.13）,t值为3.395与2.002,P均〈0.05。结论 小茴香热熨用于肝硬化门奇断流术后胃肠功能恢复有确切的疗效。Objective To discuss the effects of fennel ironing method on gastrointestinal function recovery after portal-azygous disconnection for treating hepatocirrhosis. Methods 59 samples with portal-azygous disconnection for treating hepatocirrhosis were divided into the treatment group and control group. The treatment group and the conventional group were given the conventional preoperative nursing （lying in bed, reasonable diet, symptomatic and hepatoprotective treatment, hemostasis, diuresis, nutritional support, paying attention to the treatment of complications and health education） and conventional postoperative nursing （lying in bed, paying attention to posture, from the liquid diet to the normal diet, close observation and timely feedback and recording, prevention of complications and health education）. In addition, fennel ironing method （sterile gauze was covered on the Shenque point, and the medicinal bag for ironing was pressed on Shenque point, then taking this as the center, the abdomen was massaged clockwise） was given to the treatment group, while TDP radiation of abdomen （12h after the operation, TDP was used to radiate the abdomen for 15-20min, twice a day） was given to the control group. Results The postoperative recovery time of bowel sounds of the treatment group is （48.33±13.68）, earlier than （60.27±13.32） of the control group （t=3.395; P〈0.05）. And the recovery time for passage of gas by anus of the treatment group is（70.76±15.32）, earlier than （82.37±15.13） of the control group（t=2.002; P〈0.05）. Concluslon The effects of fennel ironing method on gastrointestinal function recovery after portal-azygous disconnection for treating hepatocirrhosis are clear.
