环氧化物酶-2与缺氧诱导因子-1α在肺癌组织中的表达及临床意义

摘  要:目的探讨环氧化物酶—2(cyclooxygenase2,COX—2)与缺氧诱导因子-1αhypoxia-inducible factor1,HIF—1α)在肺癌组织中的表达及临床意义。方法随机选取2011年5月—2013年5月在该院治疗的56例肺癌患者的肺癌组织标本,进行病理学检查,其中小细胞癌18例,腺癌9例,鳞癌29例;早期(Ib期,IIIa期)26例,晚期(IIIb期,IV期)30例。应用免疫组化法对56例肺癌组织中的COX-2、HIF-1α的表达进行检测;用原位核酸分子杂交方法检测H IF^1在非小细胞肺癌癌组织中的表达;同时用CD34单克隆抗体对血管内皮细胞进行标记并计数微血管密度(MVD),对COX-2、HIF-1α与肺癌组织及其血管生成的关系进行统计学分析。结果 156例肺癌组织内COX-2的阳性表达为14例(78.57%),其中在小细胞癌中阳性表达为14例(77.78%),腺癌阳性表达为8例(88.89%),鳞癌阳性表达率22例(75.86%),COX-2阳性表达率不受病理分型影响,差异无统计学意义(P>0.05);在早期(Ib-IIIa期)肺癌组中COX-2阳性表达率为61.54%,晚期(IHb-IV期)表达率为90.00%,COX-2阳性表达率有病理分期有关,差异有统计学意义(P<0.05)。256例肺癌组织内HIF-1α阳性表达为45例(89.49%),其中在小细胞癌中阳性表达为17例(94.44%),腺癌阳性表达为7例(77.78%),鳞癌阳性表达为25例(86.21%),HIF-1α阳性表达率不受病理分型影响,差异无统计学意义(P>0.05);在早期(Ib-IIIa期)的肺癌组中HIF-1α阳性表达率为80.77%,晚期(IIIb-IV期)为93.33%,HIF-1α有病理分期有关,差异有统计学意义(P<0.05)。3COX-2弱阳性的MVD值为13.32±8.91,较强阳性为19.20±9.98,强阳性为23.04±5.69,在病灶部位MVD随着COX-2表达强度增加而增加,差异有统计学意义(P<0.05);HIF-1α弱阳性MVD值为13.78±4.59,较强阳性为21.59±8.25,强阳性为28.58±2.59,在病灶部位MVD随着HIF-1α表达强度增加而增加,差异有统计学意义(P<0.05)。结论 COX-2、HIF-1α对于肺癌组织及其血管的生成具有重要作用,探讨分析Objective To investigate the expression of cyclooxygenase-2 (COX-2) and hypoxia-inducible factor-1α (HIF-1α) in lung cancer tissue and its clinical significance. Methods The lung cancer specimens of 56 cases with lung cancer treated in our hospital from May 2011 to May 2013 were randomly selected for pathological examination, the result showed that there were 18 cases with small cell carcinoma, 9 cases with adenocarcinoma, 29 cases with squamous cell carcinoma; 26 cases were early stage (Ib stage, IIIa stage), 30 cases were late stage(IIIb stage, IV stage), lmmunohistochemical method was used to detect the expression of COX-2 and HIF-1α in 56 cases of lung cancer tissues; in situ hybridization method was used to detect the expression of HIF-1α in non-small cell lung cancer tissue, CD34 monoclonal antibody was used to label the vascular endothelial ceils and microvessel density (MVD) was counted; the correlation of COX-2, HIF-1α with lung cancer tissue and angiogenesis was analyzed statistically. Results (1) Of the 56 cases of lung cancer tissues, there were 14 cases(78.57%) with positive expression of COX-2; 14 cases (77.78%) with positive expression in small cell carcinoma, 8 cases (88.89%) with positive expression in adenocarcinoma, 22 cases' positive expression rate of squamous cell carcinoma was 75.86%; the positive expression rate of COX-2 was not affected by the pathological type(P〉O.05); The positive expression rate of COX-2 was 61.54% in the early(Ib-IIIa stage) lung cancer group, and 90.00% in the late(IHb-IV stage) lung cancer group, the expression rate of COX-2 was correlated with the pathological stage (P〈 0.05). (2) Of the 56 cases of lung cancer tissues, there were 45 cases (89.49%) with positive expression of HIF-1α, 17 cases (94.44%) with positive expression in small cell carcinoma, 7 cases (77.78%) with positive expression in adenocarcinoma, 25 cases (86.21%) with positive expression in squamous cell carcinoma, th
