摘　　要：目的：评估早孕期规范化超声检查在诊断胎儿畸形中的应用价值。方法：选取2011年1月～2013年12月在该院做颈部透明带（NT）检查的孕11～13＋6周孕妇10 976例,对其分娩的10 996个胎儿进行规范化超声检查,并对异常病例追踪妊娠结局。结果：检出235例共340个异常,异常检出率为2.14%,其中结构异常79例、单纯NT增厚99例、其他异常57例。137例患者选择终止妊娠,98例患者选择继续妊娠,继续妊娠患者中58例中晚期超声检查未发现结构异常,40例出现新的异常;74例行染色体检查,检出染色体异常10例,异常检出率为13.51%。结论：早孕期规范化超声检查可筛查出部分严重结构异常及染色体异常软指标,为指导产前介入诊断提供依据,但其并不能取代中孕期胎儿超声检查。Objective： To evaluate the application value of standardized uhrasonography during the first trimester of pregnancy in diagnosis of fetal malformations. Methods： A total of 10 976 pregnant women of 11 - 13 ＋6 gestational weeks receiving nuchal translucency （NT） examination in the hospital from January 2011 to December 2013 were selected, then 10 996 fetuses boru by these mothers underwent standardized uhrasonography, the pregnancy outcomes of abnormal cases were followed up. Results ： A total of 340 abnormalities in 235 ca- ses were detected, the detection rate was 2. 14% , 79 cases with structural abnormalities, 99 cases with simple NT thickening and 57 cases with other abnon＇nalities were included. A total of 137 cases chose to terminate pregnancy; 98 cases chose to continue pregnancy, then 58 ca- ses were not found with structural abnormalities by uhrasonography during the second and the third trimesters of pregnancy, 40 cases were found with new abnormalities; 74 cases underwent chromosomal examination, 10 cases of them were found with chromosomal abnormalities, the detection rate was 13.51%. Conclusion： Standardized uhrasonography during the first trimester of pregnancy can screen out partial seri- ous structural malformations and abnormal chromosomal soft indexes, which provides a basis for guiding prenatal interventional diagnosis, but it does not replace fetal ultrasound screening during the second trimester of pregnancy.
