腹腔镜右半结肠切除术的临床疗效分析

摘　　要：目的探讨比较腹腔镜辅助右半结肠切除术和传统开放性切除手术的疗效。方法回顾性分析2010年1月—2013年10月在河南大学淮河医院69例行腹腔镜右半结肠切除术和同期78例行开腹右半结肠切除术患者的临床资料。比较两组临床资料、手术相关资料、术后近期疗效等的差异。结果两组病人临床资料间的差异无统计学意义;腹腔镜组切口长度、手术时间、术中出血量、术后肛门排气时间、术后住院天数,分别为(5.3±0.7)cm,(202.8±16.2)min,(84.8±43.0)mL,(2.5±0.6)d,(10.0±2.0)d,与开腹组相比(21.3±2.5)cm、(168.0±16.6)min、(203.3±35.3)mL、(3.9±0.7)d、(15.9±5.1)d)差异有统计学意义(P<0.05),而开腹组清扫淋巴结数目(12.9±4.5)个,腹腔组清扫淋巴结数目(16.4±7.6)个,两组清扫淋巴结数目差异无统计学意义(P>0.05)。结论应用腹腔镜辅助右半结肠切除术的近期疗效优于传统开腹手术,有利于患者术后的康复和生活质量的提高。Objective To observe and compare the efficacy between laparoscopic-assisted right hemicolectomy（LR） and traditional open right hemicolectomy（OR） for colon carcinoma. Methods The clinical data of 69 cases treated by LR and 78 cases by OR in Henan University Huaihe Hospital from January 2010 to October 2013 were analyzed retrospectively. The clinical data, data about operation, and the short-term efficacy after the operation of the two groups were compared. Results There are no differences between the two groups in clinical data. The length of incision, duration of operation, intraoperative blood loss, the mean time of anal aerofluxus, and the mean postoperative hospital stay of the laparoscopic group was（5.3 ±0.7）cm,（202.8±16.2）min,（84.8±43.0）ml,（2.5±0.6） d,（10.0±2.0） d, respectively. Compared with those of the laparotomy group（21.3±2.5 cm, 168.0±16.6 min, 203.3±35.3ml, 3.9±0.7 d, 15.9±5.1 d）, the differences had statistical significance（all P〈0.05）. However, the number of lymph nodes dissection of the laparotomy group were（12.9±4.5）, and that of the laparoscopic group were（16.4±7.6）, the difference between the two groups was not statistically significant（P〉0.05）. Conclusion The laparoscopic-assisted right hemicolectomy is superior to traditional open surgery in short-term efficacy. LR is contributed to the recovery from surgery and the improvement of the quality of life.
