摘 要:目的探讨母牛分支杆菌菌苗联合常规化疗对初治菌阳肺结核的临床疗效。方法将2012年7月—2013年7月来该中心进行就诊的痰菌初阳肺结核患者120例随机分为治疗组60例和对照组60例。两组抗结核化疗方案相同。治疗组从化疗开始的第1周末加用母牛分支杆菌菌苗(微卡菌苗)22.5μg深部肌内注射,每两周1次,疗程6个月;观察组仅单用化疗方案。研究两组痰菌阴转情况、病灶吸收情况、空洞改变情况及不良反应。结果治疗组2个月末及3个月末的痰菌阴转率分别为83.3%及96.6%,高于观察组的61.7%及78.3%,差异有统计学意义(P<0.05);疗程结束时,治疗组的病灶改善率为96.7%优于观察组的83.3%,差异有统计学意义(P<0.05)。疗程结束时,治疗组的空洞关闭率为89.2%高于对照组的76.9%,差异有统计学意义(P<0.05)。结论母牛分支杆菌菌苗联合常规化疗治疗初治菌阳肺结核的临床疗效较好,痰菌转阴率高,病灶吸收情况好,且不良反应轻微,值得推广应用。Objective To observe the clinical effect of mycobacterium vaccae vaccine combined with the conventional chemotherapy on new sputum smear positive pulmonary tuberculosis. Methods 120 new sputum smear positive patients with pulmonary tuberculosis visited our center from July, 2012 to July, 2013 were randomly divided into the treatment group of 60 cases and the control group of 60 cases. Both groups were treated with the same anti-tuberculosis chemotherapy. In addition, from the first weekend of the chemotherapy, the treatment group were given deep intramuscular injection of mycobacterium vaccae vaccine(vaccae vaccine),22.5μg, one time every two weeks, for a course of treatment of 6 months. The observation group were only given chemotherapy.The negative conversion of the sputum, focus absorption situation, change of cavity and adverse reactions of the two groups were studied. Results At the end of the second month and the third month of treatment, the sputum negative conversion rate of the treatment group was 83.3%, 96.6%, respectively, higher than 61.7%, and 78.3% of the observation group, respectively, the differences were statistically significant(P〈0.05). At the end of the treatment, the lesions improvement rate of the treatment group was96.7%, better than 83.3% of the observation group, the difference was statistically significant(P〈0.05); the cavity closure rate of the treatment group was 89.2%, higher than 76.9% of the control group, the difference was statistically significant(P〈0.05). Conclusion The clinical efficacy of mycobacterium vaccae vaccine combined with conventional chemotherapy in the treatment of new sputum smear positive pulmonary tuberculosis is better with higher sputum negative conversion rate, better focus absorption and milder adverse reactions, which is worthy of popularization and application.
