短效静脉麻醉药在初次妊娠无痛人流术中应用的研究

摘　　要：目的探讨静脉麻醉联合宫颈局麻药物与单一静脉麻醉药物在初次妊娠无痛人流术中的比较。方法 120例初次无痛人流术患者随机分为3组,A组单一使用丙泊酚;B组芬太尼-丙泊酚联合用药;C组芬太尼-丙泊酚-宫颈口利多卡因局部注射联合用药。评价3种方案的安全性和有效性。结果 A组的麻醉镇痛效果为92.5%,宫颈松驰率为68.5%;B组和C组的麻醉镇痛效果均为100%,而宫颈松驰率前者为72.2%,后者为89.6%,联合组麻醉镇痛效果优于单一使用丙泊酚(P<0.5)。术后安全性方面,A组苏醒时间(6.46±1.05)分、定向力恢复时间(10.4±2.05)分,较B组(6.46±1.05)分及C组(10.4±2.05)分要长,两者差异有统计学意义(P<0.5),且人流综合征发生率A组(3.75%)高于B组及C组(0%)。结论静脉麻醉联合宫颈局麻是一种安全、有效的方法。Objective To compare the effect of intravenous anesthesia combined with cervical local anesthesia and single intravenous anesthesia applied in the first pregnancy painless abortion. Methods 120 patients with first pregnancy painless abortion were randomly divided into three groups, group A was treated with single propofol; group B with fentanyl and propofol; group C with fentanyl, propofol combined with lidocaine injected in the cervix. And the safety and effect of these three schemes were evaluated. Results The effect of anesthesia and analgesia of group A was 92.5%, and that of the group B and group C was 100%, the cervical relaxation rate of group A was 68.5%, and that of group B and group C was 72.2%, 89.6%, respectively, the effect of anesthesia and analgesia of combined group was better than that of the single propofol（P〈0.5）. In the aspect of postoperative safety,the awakening time of group A was（6.46±1.05） min, orientation recovery time was（10.4±2.05） min, longer than（6.46±1.05） min of group B and（10.4±2.05） min of group C, the difference was statistically significant（P〈0.05）; and the incidence of abortion syndrome of group A was 3.75%, higher than 0% of group B and group C. Conclusion Intravenous anesthesia combined with cervical local anesthesia is a safe and effective method.
