摘  要:目的:研究幼儿EB病毒感染不同临床类型免疫功能的变化特点,为EB病毒感染的防治提供研究资料。方法:196例EB病毒感染患儿根据临床类型分为IM组124例,CAEBV组57例,EBV-HLH组15例,并以69例健康儿童为对照组,比较各组细胞免疫及体液免疫功能变化。结果:IM组CD3+T细胞出现升高,EBV-HLH出现降低(P<0.05);IM组、CAEBV组及EBVHLH组CD4+T细胞均出现降低(P<0.05),其中以EBVHLH组下降程度最高;IM组CD8+T细胞出现升高,与其他各组比较有统计学意义差异(P<0.05);IM组及EBVHLH组CD4+/CD8+比值出现降低,与其他各组差异有统计学意义(P<0.05);IM组、CAEBV组及EBVHLH组CD19+细胞均出现降低,与对照组比较差异有统计学意义(P<0.05);EBVHLH组CD56+细胞出现降低,与其他各组比较有统计学差异(P<0.05);EBVHLH组Ig G出现降低,与其他各组差异有统计学意义(P<0.05);IM组、CAEBV组Ig M出现升高,EBVHLH组出现降低,差异均有统计学意义(P<0.05)。结论:EB病毒感染导致的IM、CAEBV及EBVHLH 3种疾病发病机制及疾病的进展均与免疫功能紊乱有关,对患者免疫功能,尤其是细胞免疫的监测有重要的临床意义。Objective: To study the change characteristics of immune function in children with different clinical types of EB virus infection, provide research data for prevention and treatment of EB virus infection. Methods: A total of 196 children with EB virus infection were divided into IM group (124 children) , CAEBV group (57 children) and EBV -HLH group (15 children) . Sixty -nine healthy children were selected as control group; the changes of cellular immunity and humoral immune in the four groups were compared. Results: CD3 + T cells increased in IM group and decreased in EBV - HLH group ( P 〈0.05 ) ; CD4 + T ceils in IM group, CAEBV group and EBV - HLH group decreased ( P 〈 0. 05 ) , especially in EBV - HLH group ; CD8 + T cells in IM group increased, compared with the other groups, there were statistically significant differences (P 〈 0. 05 ) ; CD4 +/CD8 + ratios in IM group and EBV - HLH group decreased, compared with the other groups, there were statistically significant differences (P 〈 0. 05 ) ; CD19 + cells in IM group, CAEBV group and EBV -HLH group decreased, compared with control group, there were statistically significant differences ( P 〈 0. 05 ) ; CD56 + cells in EBV - HLH group decreased, compared with the other groups, there were statistically significant differences ( P 〈 0. 05 ) ; IgG level in EBV - HLH group decreased, compared with the other groups, there were statistically significant differences ( P 〈 0. 05 ) ; IgM levels in IM group and CAEBV group increased, [gM level in EBV - HLH group decreased, there were statistically significant differences (P 〈 0. 05) . Conclusion:The pathogenesis and development of IM, CAEBV and EBVHLH induced by EB virus infection are correlated with immune dysfunction, which has important clinical significance for monitoring immune function of the patients, especially cellular function.
