摘 要:目的 探讨自身抗体与狼疮肾炎(lupus nephritis,LN)病理类型的相关性.方法 采用免疫印迹法测定75例行肾活检LN患者的抗dsDNA、抗Sm、抗SSA、抗U1nRNP、抗核小体(NUC)及抗组蛋白(HIS)抗体,并与56例无肾脏损害的系统性红斑狼疮(systemic lupus erythematosus,SLE)患者对照,分析自身抗体与LN病理类型的关系.结果 75例LN患者中,抗dsDNA、抗Sm和抗HIS阳性率均明显高于无肾脏损害患者(P<0.05);75例LN病理类型分属Ⅰ~Ⅵ型,与无肾脏损害组对照,抗dsDNA和抗HIS抗体阳性率在各型LN中均增高,但仅Ⅱ型LN差异有统计学意义(P<0.01),抗SSA抗体在Ⅰ型LN中与无肾脏损害组对照,差异有统计学意义(P<0.05),抗U1nRNP抗体在Ⅴ型LN中与无肾脏损害组对照,差异有统计学意义.结论 抗dsDNA抗体、抗Sm抗体和抗HIS抗体阳性可指导LN的诊断,且抗dsDNA抗体和抗HIS抗体与Ⅱ型LN发病相关,抗SSA抗体与Ⅰ型LN发病有关,抗U1nRNP抗体则可能在Ⅴ型LN病变中发挥作用.Objective To analyze the association between anti-extractable nucler antigen (ENA) and the histological changes in luPus nephritis(LN) .Methods The anti-dsDNA,anti-SSAI,anti- U 1 nRN P/Sm, anti-nucleosome ( N UC), and anti-histone antibody were detected in 75 patients with LN underwent renal biopsy and 56 patients without LN by using Western-blot assay,followed by analyzeing the relations between the anti-ENA autoantibodies and LN pathological changes.Results In 75 patients with LN,anti-dsDNA antibodies, Sm and anti-histones (HIS) antibodies positive rate were significantly higher than those of patients without kidney damage (P〈0.01) ;75 cases of LN pathological type belong to Ⅰ -Ⅵ type,compared with no kidney damage control group,anti-dsDNA antibodies and protein antibody positive rate in each model LN resistance group were increased,but only type Ⅱ LN difference was statistically significant (P〈0.01),and anti-SSA antibody in type Ⅰ LN compared with no kidney damage control group,the difference was statistically significant (P〈0.05).Conclusion Anti-dsDNA antibodies, anti-Sm and anti-histones antibody positive can guide the diagnosis of LN ,and dsDNA antibodies associated with the onset of type Ⅱ LN,anti-SSA antibody associated with the onset of type Ⅰ LN,and anti-U1nRNP antibodies may play a role in the type V LN pathological changes.
