摘  要:目的 比较乳腺癌改良根治术后三种负压引流方法的效果。方法 回顾性分析同济大学附属杨浦医院乳腺外科2002年9月~2010年3月期间住院接受乳腺癌改良根治手术治疗,术后分别采用墙式负压吸引、胃肠减压器或硅胶负压引流球的乳腺癌患者共180例。结果 乳腺癌改良根治术后使用墙式负压吸引的患者拔管较早,且皮下积液的并发症发生率最低,使用负压球引流的患者拔管最晚,皮下积液的发生率最高。体重指数小于22.5的患者,术后采用胃肠减压器或硅胶负压引流球负压引流效果相似;体重指数大于22.5的患者,术后采用胃肠减压器负压引流效果优于硅胶负压引流球。结论 墙式负压吸引使用效果好,积液并发症少,但装置复杂,限制患者活动。胃肠减压器临床使用效果略优于硅胶负压引流球,对于BMl〉22.5的患者,术后应尽量采用一次性胃肠减压器引流,以缩短引流管置管时间,减少皮下积液并发症的发生。Objective To compare the three kinds of negative pressure drainage effect after modified radical mas‐tectomy for breast cancer .Method 180 patients with mastectomy from September 2002 to March 2010 in Yangpu hospital was surveyed by retrospective analysis method .The patients was given different drainage method including wall negative pressure suction ,gastrointestinal device and silicon negative pressure drainage ball .Result The patients used wall negative pressure suction after mastectomy had shortest time of indwelling drainage and the least complica‐tion .The patients used silicon negative pressure drainage ball ,on the contrary had the most complication .Gastroin‐testinal decompression device is most effective than silicon negative pressure ball for patients whose BMI less than 22 .5 .There was no significant difference for patients whose BMI more than 22 .5 .Conclusion Wall negative pressure suction is the most effective method for patients .But it would restrict patients activity .Gastrointestinal decompres‐sion devices is better than silicon negative pressure drainage ball .for patient whose BMI more than 22 .5 ,we should use gastrointestinal decompression devices in order to shorten the time of drainage and reduce the incidence of subcu‐taneous fluid .
