摘 要:目的:通过检测2型糖尿病患者脂蛋白及尿微量蛋白水平,为糖尿病肾病的早期诊断与预防提供依据。方法:按病程将糖尿病肾病病人分成三组:采用全自动生化分析仪检测空腹血糖、糖化血红蛋白(HbA1c)、血浆脂蛋白、肾功能及尿微量蛋白水平。结果:B组及C组病人之和占总糖尿病肾病病人71.43%,且年龄较大(P<0.05);病程较短的糖尿病肾病病人空腹血糖高于其它各组(P<0.01),TG、LDL-C、APOB及LPA高于病程较长组,且三组都低于单纯糖尿病组,而肾功能各项及尿微量蛋白均高于单纯糖尿病组(P<0.01),CysC水平无统计学意义(P>0.05)。结论:糖尿病肾病患者存在糖及脂蛋白代谢紊乱,尿微量蛋白结合血脂检测有利于早期发现肾脏损伤。Objective:To supply the basis tor early diagnosis and prevention oi type 2 diabetic nephropathy by detecting the lebel of plasma lipoprotein and urine minim protein (UMPR) in patients. Methods:The patients were di- vided three groups by the course of diabetic: The fasting blood-glucose (FBG), HbAlc, plasma lipoprotein, BUN, Scr, UA, CysC and UMPR were detected by automatic biochemical analyzer. Results:The sum of A and B groups was 71.4% of total, and patients were older (P〈0.05); The level of FBG in A was higher than other groups (P〈0.01), and TG, LDL-C, APOB, LPA were higher than longer course groups also, but they all were lower than pure diabetic; Compared with pure diabetic group, the levels of BUN, Scr, UA, and UMPR were higher (P〈0.01), but CysC was no statistically significant (P 〉0.05). Conclusion:There are glucose and lipoprotein metabolism disorder in patients with diabetic nephropathy, detection of UMPR and plasma lipoprotein is beneficial to early detection of kidney damage.
