摘  要:目的探讨行IVF-ET妇女体内颗粒细胞线粒体活性与血浆、卵泡液左旋肉碱水平三者的关系,及其与年龄、优胚率、受精率及妊娠结局等的关系。方法应用流式细胞仪法测定颗粒细胞线粒体活性,柱前衍生-高效液相色谱法测定游离左旋肉碱水平。结果妊娠组颗粒细胞线粒体活性高于未妊娠组(P<0.05)。颗粒细胞线粒体活性与年龄呈负相关,与受精率、正常受精率、优胚率呈正相关(P<0.05)。妊娠组卵泡液左旋肉碱水平低于未妊娠组(P<0.05),血浆左旋肉碱水平与年龄呈正相关(P<0.05),血浆左旋肉碱水平与颗粒细胞线粒体活性呈负相关(P<0.05)。结论颗粒细胞线粒体活性的降低可能与年长妇女IVF-ET受精率低、胚胎质量差、妊娠结局差有关。妇女体内颗粒细胞线粒体活性下降时,血浆左旋肉碱水平可能代偿性增加。【Objective】To investigate the correlation of mitochondrial activity in the granulosa cells with plasma and follicular fluid carnitine level on the in vitro fertilization-embryo transfer(IVF-ET),and to examine the correlation among the three parameters and age, fertilization, cleavage, good-embryo rates and themselves.【Methods】The Free L-carnitne level and the mitochondrial activity were measured.【Results】Pregnant patients showed significantly higher mitochondrial activity(P〈0.05) in granulosa cells, lower follicular fluid L-carnitine level(P〈0.05). A declined mitochondrial performance had been observed during aging(P〈0.05), which has positive correlation with fertilization rate, normal fertilization rate and good-embryo rate(P〈0.05). A positive correlation was seen between plasma L-carnitine level and age(P〈0.05). A negative correlation was seen between L-carnitine level and mitochondrial activity(P〈0.05). 【Conclusion】The mitochondrial activity in granulosa cells may effect the development of oocytes and embryos and therefore lead to poor outcome of IVF-ET of elder woman. High L-carnitine level in plasma may compensatory increase when the mitochondrial activity in granulosa cells has decreased.
