α-硫辛酸联合莫沙必利治疗糖尿病胃轻瘫临床观察

摘　　要：目的观察α-硫辛酸联合莫沙必利治疗糖尿病胃轻瘫的疗效。方法选取2011年10月至2012年12月在河北港口集团港口医院住院的有上消化道症状的糖尿病胃轻瘫患者80例,采用随机数字表将患者分成两组:α-硫辛酸和莫沙必利联合组40例(α-硫辛酸0.6 g+0.9%NaCl溶液250 mL避光静脉滴注14 d+莫沙必利5 mg饭前口服,每日3次),莫沙必利组40例(莫沙必利5mg饭前口服,每日3次),治疗2周,通过餐后上腹饱胀、早饱、厌食、恶心呕吐等临床症状观察及胃排空率判断疗效,并检测胃动素水平的变化。结果两组在改善临床症状上均有效,α-硫辛酸和莫沙必利联合组改善临床症状的总有效率为92.0%,莫沙必利组改善临床症状的总有效率为72.5%。α-硫辛酸和莫沙必利联合组胃排空率较莫沙必利组显著提高(P<0.05);两组治疗后胃动素均有下降:α-硫辛酸和莫沙必利联合组治疗前后胃动素分别为(408.35±23.06)ng/L、(198.53±18.16)ng/L,莫沙必利组治疗前后胃动素分别为(398.24±27.35)ng/L、(320.67±31.45)ng/L(P<0.05)。结论对于糖尿病胃轻瘫患者,α-硫辛酸和莫沙必利联合用药能有效改善胃轻瘫症状、提高胃排空率,且疗效优于莫沙必利单药治疗。Objective To observe the efficacy ofα-lipoic acid mosapride treating diabetic gastroparesis.Methods A total of 80 patients with diabetic gastroparesis who has gastrointestinal symptoms admitted to Port Hospital of Hebei Port Group during Oct.2011 and Dec.2012 were included,in this study.They were randomly divided into two groups,40 cases in α-lipoic acid and mosapride combination therapy group （α-lipoic acid 0.6 g ＋ 250 mL 0.9％ saline intravenously 14 d ＋ mosapride 5 mg orally before meals,three times a day）,and 40 cases in mosapride therapy group （mosapride 5 mg orally before meals,three times a day）.After two weeks of treatment,through observation of clinical symptoms of postprandial abdominal fullness,early satiety,anorexia,nausea and vomiting and determining the gastric emptying rate,the efficacy was judged,and the changes in the level of motilin were detected.Results Effects in improving clinical symptoms were observed in both groups：the total effective rate of α-lipoic acid and mosapride combination therapy group was 92.0％ ;of the mosapride therapy group was 72.5％ ; Gastric emptying rate ofα-lipoic acid and mosapride combination therapy group was more significantly increased than mosapride therapy group（P ＜0.05） ; the motilin level was reduced after treatment in both gruops.Motilin level in α-lipoic acid and mosapride combination therapy group before and after treatment were （408.35 ± 23.06） ng/L,（198.53 ± 18.16） ng/L respectively,of mosapride therapy group before and after treatment motilin were （398.24 ± 27.35） ng/L,（320.67 ± 31.45） ng/L respectively （P ＜ 0.05）.Conclusion For patients with diabetic gastroparesis,α-lipoic acid and mosapride combination therapy can improve symptoms of gastroparesis and improve gastric emptying rate,which is more effective than monotherapy of mosapride.
