胺碘酮联合稳心颗粒治疗慢性心力衰竭患者室性心律失常的临床观察

摘　　要：目的观察胺碘酮联合稳心颗粒治疗慢性心力衰竭(CHF)合并室性心律失常(VA)患者的临床疗效。方法将2008年6月至2012年4月在唐山市人民医院住院的60例CHF合并VA患者按照随机数字表法分为两组:胺碘酮组(30例)给予胺碘酮口服0.2 g,每日3次;胺碘酮联合稳心颗粒组(30例)在胺碘酮组的基础上给予稳心颗粒9 g口服,每日3次。评价两组患者用药4周后的疗效和不良反应情况。结果胺碘酮组失访5例,胺碘酮组的有效率为84.0%(21/25),胺碘酮联合稳心颗粒组的有效率为83.3%(25/30),两组比较差异无统计学意义(χ2=0.09,P>0.05)。但胺碘酮组的不良反应发生率为23.3%,胺碘酮联合稳心颗粒组为6.7%,两组比较差异有统计学意义(χ2=4.32,P<0.05)。结论单用胺碘酮和胺碘酮联合稳心颗粒治疗CHF合并VA患者的疗效均较好,但联合用药组的不良反应少。Objective To observe the clinical effect and safety of wenxin granule in treating patients with chronic heart failure（CHF） accompanied with ventricular arrhythmia （VA）.Methods A total of 60 cases of patients with CHF accompanied with VA admitted in Tangshan People＇s Hospital from Jun.2008 to Apr.2012 were randomly divided into two groups according to the random number table,with 30 cases in each group：patients in amiodarone group orally taken amiodarone 0.2 g,three times per day;patients in the combination group orally taken wenxin granule 9 g,threc times per day on the basis of the therapy of amiodarone group.After 4 weeks of treatment,the clinical effects and adverse reactions of the two groups were evaluated.Results There were 5 cases lost in follow-up in amiodarone group.The total effective rate of the amiodarone group was 84.0% （21/25）,and the total effective rate of the combination group was 83.3% （25/30）,there was no statistically significant difference between the two groups（Х^2 =0.09,P 〉0.05）.The adverse reaction of the amiodarone group was 23.3%,and that of the combination group was 6.7%,there was statistically significant difference between the two groups （Х^2 =4.32,P 〈 0.05）.Conclusion The curative effect of either amiodarone or wenxin granule combined with amiadarone in treating patients with CHF accompanied with VA is good,while the combined therapy has less adverse reactions.
