针刀对腰突根性神经痛大鼠外周镇痛机制的实验研究

摘　　要：目的观察针刀疗法对腰椎间盘突出症根性神经痛大鼠血清中5-羟色胺、β-内啡肽含量的影响,探讨针刀疗法对腰椎间盘突出症根性神经痛干预作用的可能机制。方法将40只雄性SD大鼠随机分为正常组、假模型组、模型组、针刀组,每组10只。用热痛测试仪检测各组大鼠造模前后的缩爪潜伏期;HE染色观察L5神经根的病理学变化;造模后第8天分别取各组大鼠血清,采用双抗夹心ELISA方法检测5-HT、β-内啡肽的含量。结果各组大鼠血清中5-HT含量的比较,模型组(12.19±1.15)ng/mL较正常组(10.40±1.10)ng/mL显著升高,差异有统计学意义(P<0.01),针刀组(10.73±1.11)ng/mL较模型组(12.19±1.15)ng/mL显著降低,差异有统计学意义(P<0.01);各组大鼠血清中β-EP含量的比较,模型组(108.14±11.08)ng/L较正常组(85.60±15.13)ng/L显著升高,差异有统计学意义(P<0.01),针刀组(90.76±15.58)ng/L较模型组(108.14±11.08)ng/L显著降低,差异有统计学意义(P<0.01)。结论提示针刀疗法可通过对血清中5-羟色胺、β-内啡肽含量的调节作用,减缓腰椎间盘突出症根性神经痛的无菌性炎症反应,发挥局部镇痛作用。Objective To observe the effect of acupotomy on 5-HT and β-EP in serum of Rats with radicular pain of lumbar disc herniation,so as to explore possible mechanism of acupotomy.Methods Forty male SD Rats were randomly divided into normal group,sham model group,model group,acupotomy group,four groups in all and 10 Rats in each group.The incubation period of shrinkage claw of each group was tested by PL-200;The pathological changes of L5 nerve root were observed by HE dyeing;Samples were took on eighth day after model,and the serum were taken to observe the changes of 5-HT and β-EP by ELISA.Results The comparison of the 5-HT among each group was that 5-HT of the model group（12.19 ± 1.15） ng /mL in serum increased significantly（P〈0.01） compared with normal group（10.40 ±1.10） ng/mL;acupotomy group（10.73 ±1.11） ng/mL decreased compared with model group（12.19 ±1.15） ng /mL（P〈0.01）.The comparison of the β-EP among each group was that β-EP of the model group（108.14 ± 11.08） ng /L in serum increased significantly（P〈0.01） compared with normal group（85.60 ± 15.13） ng /L;acupotomy group（90.76 ± 15.58） ng /L decreased compared with model group（108.14 ± 11.08） ng /L（P〈0.01）.Conclusion Acupotomy has a benign effect on regulating 5-HT and β-EP in serum with lumbar disc herniation.
