联合血液净化治疗老年透析患者顽固性高血压

摘　　要：目的观察老年透析患者顽固性高血压的治疗效果。方法将42例患者随机分为二组,每组21例。A组患者每周2次常规血液透析加每周1次血液透析滤过;B组患者每周2次常规血液透析加每周1次血液透析滤过加二周一次血液灌流治疗,观察治疗前、后二组患者血压变化、血浆内皮素(ET)、血管紧张素Ⅱ(ATⅡ)、肾素(RA)水平、甲状旁腺素(PTH)水平。结果二组患者治疗后血压明显下降、ET、ATⅡ、RA、PTH均降低,与治疗前相比差异有统计学意义,组间比较差异有统计学意义。结论联合血液净化治疗有助于提高老年透析患者的生存质量。Objective To observe the curative effect of elderly hemodialysis patients with refractory hypertension.Methods Forty-two patients were randomly divided into two groups,21 cases in each group.Patients in group A were treated by conventional hemodialysis 2 times a week and hemodiafiltration 1 time per week;patients in group B was treated by conventional hemodialysis 2times a week,hemodiafiltration1 times a week and blood perfusion treatment once every two weeks.The blood pressure,the changes of plasma endothelin（ET）,plasma angiotensinⅡ（ATⅡ）,renin activity（RA）,the level of parathyroid hormone（PTH）were observed before and after treatment in two groups.Results Compared with before treatment,the blood pressure decreased significantly,ET,ATⅡ,RA,PTH of two groups were decreased after treatment,the difference was statistically significant and there was also statistically significant difference between groups.Conclusion Combine with blood purification therapy is helpful to improve the quality of life of elderly dialysis patients.
