复方右旋糖酐40注射液致过敏性休克

摘　　要：1例28岁孕妇,孕39+6周,因羊水少给予复方右旋糖酐40注射液500 mL静脉滴注,输注5 min后患者出现全身皮肤发红、皮疹,意识清楚,对答切题,口唇发绀,桡动脉未扪及搏动,血压未检测到。立即停止用该药,并给予吸氧、升压、抗过敏、抗休克等对症治疗,90 min后该患者上述症状缓解,但胎死宫内。6 d后患者一般情况良好,生命体征平稳无并发症,予以出院。One 28-year-old pregnant woman who was at 39＋ 6 weeks of gestation, received the intravenous drip of compound dextran 40 injection 500 mL because of oligohydramnios. After 5 minutes, the patient developed systemic skin redness, rash, oral cyanosis. The radial artery pulse and blood pressure could not be detected, while the patient could answer questions with clear consciousness. The compound dextran 40 injection was stopped immediately, and the patient received symptomatic treatments, including oxygen inhalation, boosting pressure, antiallergic and antishock treatment. About 90 minutes later, the symptoms of the patient relieved, but the fetus died in intrauterine. After 6 days, the general condition of the patient was well, vital signs were stable with no complications, and the patient discharged from hospital.
