摘  要:目的了解阿米卡星、庆大霉素敏感及耐药的多重耐药鲍曼不动杆菌的氨基糖苷类修饰酶基因存在状况,揭示鲍曼不动杆菌对氨基糖苷类抗生素的耐药机制。方法采用PCR法分别检测阿米卡星(AMK)、庆大霉素(GEN)敏感株和耐药株的氨基糖苷类修饰酶基因。结果 18株对AMK、GEN耐药的氨基糖苷类修饰酶(AMEs)基因:aac(6')-I基因阳性率27.78%,aac(6')-II基因阳性率33.33%,ant(3″)-I基因阳性率77.78%,ant(2″)-I基因阳性率27.78%;10株庆大霉素、阿米卡星敏感株未检出AMEs基因。结论 18株AMK、GEN耐药株均检出氨基糖苷类修饰酶(AMEs)基因,本地区鲍曼不动杆菌以携带ant(3″)-I基因为主。实验室应加强细菌耐药性监测,指导临床合理用药。Objective To study on the distributing characterizations of aminoglycoside - modifying enzyme genes in multi - drug resistant Acinetobacter baumannii (MDRAB) and to disclose the resistant mechanisms for MDRAB against aminoglycoside anti- biotic. Methods Photo conductive relay(PCR) was performed to detect aminoglycoside - modifying enzyme genes of gentami- cin - and amikacin - sensitive as well as resistant Acinetobacter baumannii strains. Results In 18 strains of gentamicin - and amikacin - resistant Acinetobacter baumannii, the detecting rate of aac (6′) - Ⅰ, (6′) - Ⅱ, ant (3″) - Ⅰ and ant (2″) - Ⅰ were 27.78%, 33.33%, 77.78% and 27.78% respectively. The Bruce Ames(AMEs) genes were not detected in 10 strains of gen- tamicin and amikacin sensitive Acinetobacter baumannii. Conclusion All the 18 gentamicin - and amikacin - resistant Acine- tobacter baumannii strains were detected positively on the aminoglycoside - modifying enzymes (AMEs) genes, which was main- ly ant (3″) -Ⅰ gene in the region. Clinical laboratory should strengthen the surveillance of resistant strains and provide clini- cians helpful guides for rational use of antimicrobial agents.
