摘 要:目的探讨妊娠中晚期孕妇血清游离雌三醇(uE3)、游离β-绒毛膜促性腺激素(Fβ-HCG)、睾酮(T)、孕酮(Po)、促卵泡生成素(FSH)、黄体生成素(LH)六项激素水平变化及对胎儿宫内监护、胎儿预后的临床意义。方法对232例孕妇分别于妊娠14-18w、28-32w、36w-41w各采静脉血一次,采用时间分辨荧光免疫分析法和电化学发光法测定血清Fβ-HCG、uE3、T、Po、FSH、LH六项激素水平值。结果 1164例妊娠结局正常者血清游离uE3值随孕周增加逐渐升高,Fβ-HCG相对稳定,新生儿预后好;268例妊娠结局异常者血清uE3随孕周增加逐渐下降或呈持续低水平状态,且明显低于妊娠结局正常者,而血清Fβ-HCG明显高于妊娠结局正常者,发生晚期先兆流产、胎儿畸形、胎儿宫内发育迟缓、出生低体重儿、胎儿窘迫、新生儿窒息率相应升高;3正常妊娠孕妇怀男胎的血清T水平在不同孕周均高于怀女胎者,差异有显著性(P<0.05、P<0.01)。结论联合检测Fβ-HCG、uE3可作为临床诊断和筛查胎儿宫内发育迟缓、胎儿窘迫、出生低体重儿、新生儿窒息的可靠指标。Objectives: To investigate the correlation between pregnancy outcomes and the serum levels of six kinds of hormones, including follicle-stimulating hormone (FSH) , luteinizing hormone (LH) , progesterone (P) , free estriol (uE3) , free B-human chorionic gonadotropin (FB-HCG) , testosterone (T) , which were detected from the women with middle-late stage of pregnancy. The changes of the serum levels were recorded with the development of the pregnancy. Methods: 232 cases of pregnancy women were collected blood in three pregnancy periods, 14-18 week, 28-32 week, 36-41 week.The serum levels of six hormones were detected with electrochemiluminescence and time-resolved fluoroimmunoassay. Results: QIn 164 cases with nomal pregnancy outcomes the serum levels of uE3 were rised gradually with the increase of gestaional weeks, and those of the FB-HCG were relatively stable, which indicated neonatal prognosis were good: (~)In 68 cases with abnormal pregnancy outcome, serum uE3 levels declined or continued to a low status with the development of pregnancy and showed significantly difference compared to the normal pregnancy outcomes.At the same time the serum levels of FB-HCG were significantly higher than the levels of normal pregnancy outcomes. The rate of occurrence with late threatened abortion, fetal malformation, fetal intrauterine retarded, low birth weight, fetal distress and neonatal asphyxia has risen accordingly. (~)In normal pregnancy with male fetuses serum T level was significantly higher than that with female fetuses. Conclusion: Combined detection of FB-HCG and uE3 can be used as a reliable index for clinical diagnosis and screening for fetal intrauterine retardation, fetal distress, newborn aslahVxia and low birth weight.
