上海市松江区胃黏膜肠上皮化生危险因素病例对照研究

摘　　要：目的分析上海市松江地区胃黏膜肠上皮化生患者的危险因素,为肠型胃癌的防治提供依据。方法选取上海市松江区690例胃黏膜肠上皮化生患者为病例组,健康体检者1 526名为对照组,并进行问卷调查,采用病例对照研究方法,应用非条件logistic回归分析胃黏膜肠上皮化生的主要危险因素。结果多因素非条件logistic回归分析结果显示,9个因素与胃黏膜肠上皮化生的联系差异有统计学意义(P<0.01),其中年龄(OR=1.024)、使用冰箱≥10年(OR=1.255)、长期使用农药(OR=2.287)、精神紧张(OR=1.254)、饮酒≥10年(酒精摄入>25 g/d)(OR=1.954)、直系亲属患胃癌(OR=2.726)和幽门螺旋杆菌感染史(OR=1.675)是胃黏膜肠上皮化生的危险因素;而吃豆类制品>3次/周(OR=0.563)、吃水果>3次/周(OR=0.711)是其保护性因素。结论年龄、不当的生活方式和生活习惯、幽门螺旋杆菌感染史、胃癌家族史是胃黏膜肠上皮化生主要的危险因素。Objective To analyze the risk factors of gastric intestinal metaplasia in Songjiang district of Shanghai and to provide the basis of preventing and treating gastric cancer. Methods The case group （690 patients with gastric intestinal metaplasia） and control group （1 526 healthy controls） were investigated by questionnaire in Songjiang district of Shanghai. Non-conditional logistic regression was used to analyze the main risk factors of gastric intestinal metaplasia. Results The results of multiple non- conditional logistic analysis indicated that nine factors were significantly associated with gastric intestinal metaplasia （P〈0.01）. The risk factors were age （ OR= 1.024 ）, using refrigerator for ≥ 10 years （ OR = 1.255 ）, exposure to pesticides for long time （ OR =2.287 ）, stressful life （OR=1.254）, drinking for≥ 10 years （alcohol 〉25 g/d, 0R=1.954）, family history of gastric cancer （OR=2.726）, infection of H. pylori （OR= 1.675）. The protection factors were eating soy products （〉 3 times a week, OR =0.563） and eating fruits （〉 3 times a week, OR=0.711 ）. Conclusion Age, unhealthy lifestyle and dietary habits, infection of H. pylori and family history of gastric cancer are the main risk factors of gastric intestinal metaplasia.
