摘　　要：目的了解上海市杨浦区25岁以下人群伤害发生现状及社会经济负担,为预防控制提供依据。方法采用多阶段整群随机抽样的方法,对5个街道60个居委会25岁以下4 727名居民进行伤害状况调查。结果伤害的发生率为5.90%,标化率为2.58%,男性伤害发生率（7.60%）高于女性（4.21%）,差异有统计学意义﹙χ2=24.452,P〈0.01﹚,位居前5位的伤害依次是跌倒、碰撞挤压伤、扭伤、交通伤、割刺伤。0~3岁以跌倒、动物伤、烧烫伤为主,4~6岁以跌倒、碰撞挤压伤、动物伤为主,7~14及以跌倒、碰撞挤压伤、扭伤为主,15~24岁以扭伤、碰撞挤压伤、交通伤为主。发生地点居前3位的是学校、家里、运动区域。致残率为0.36%,残疾率为21.16/10万。平均每例伤害病例直接经济负担1 785.82元,人均住院1.3 d,因伤休息10.2 d,需他人看护7.5 d。结论伤害在杨浦区25岁以下人群中多发,经济负担较大,危害严重,学校、家庭及社区应积极开展儿童青少年的安全教育,采取积极有效的措施,预防和控制伤害的发生。Objective To investigate the current status and socio-economic burden of the injury in the residents younger than 25 years old in Yangpu district of Shanghai, and to provide the basis for preventing and controlling the injury. Methods The investigation of injury was conducted in 4 727 residents younger than 25 years old from 60 urban committee in 5 sub-districts by multi-stage cluster random sampling in 2010. Results Incidence of injury was 5.90% and standardized rate was 2.58%, male incidence （7.60%） was significantly higher than female incidence （4.21%, P〈0.01 ）. Five main causes of injury were fails, collision, wrench, traffic injury and sharp instrument injury. Falls, bitten by animal and burn occurred mostly in 0-3 years children. Falls, collision, and bitten by animal occurred mostly in 4-6 years children. Falls, collision and wrench occurred mostly in 7-14 years children. Wrench, collision and traffic injury occurred mostly in 15-24 years youngsters. The places where injuries occurred were mainly at schools, home and sports fields. Disability rate caused by injury was 21.16/100 000. The mean direct economic burden was 1 785.82 yuan RMB. The average hospitalization was 1.3 days. The rest duration was 10.2 days and each case with injury needed care for 7.5 days. Conclusion The injuries in youngsters younger than 25 years old frequently happen. The injuries result in big economic burden. Schools, families and communities should carry out the safety education for youngsters. The effective measures should be taken to prevent and control the injuries.
