医疗综合目标评估对医务人员手卫生正确率的作用分析

摘　　要：目的分析"医疗综合目标评估"对提升医务人员手卫生正确率的效果,以提升医院感染预防控制水平。方法 2013年1月-2014年6月在某三级甲等综合医院将包括手卫生正确率在内的医院感染管理的关键指标纳入"医疗综合目标评估"体系,并进行多种形式的反馈,通过调查,分析"医疗综合目标评估"实施前后手卫生正确率的变化。结果随着"医疗综合目标评估"的不断推进,手卫生正确率从干预前的57.21%提升至干预后期的95.30%,提升效果差异有统计学意义(χ2=1 838.04,P<0.01);除技术员在干预前后手卫生正确率的变化差异无统计学意义外,各类科室、专业人员手卫生正确率呈上升趋势并提升明显(P<0.01),至干预措施实施的Ⅲ期,手卫生正确率稳定在90.00%以上。结论采取"医疗综合目标评估"能明显提升手卫生正确率,并持续保持在较高的水平,是一项长期有效提升手卫生正确性的管理举措。OBJECTIVE To analyze the effect of comprehensive appraisal of clinical department on improvement of accuracy of hang hygiene of the health care workers so as to improve the level of prevention and control of nosocomial infections .METHODS From Jan 2013 to Jul 2014 , the key indexes for the control of nosocomial infections such as the accuracy of hand hygiene were included in the comprehensive appraisal of clinical department system in a three A general hospital ,then various feedbacks were carried out ,and the survey was conducted to observe the change of accuracy of hand hygiene before and after the comprehensive appraisal of clinical department was implemented .RESULTS With the continuous promotion of comprehensive appraisal of clinical department ,the accuracy of hand hygiene increased from 57 .21% before the intervention to 95 .30% after the intervention ,and there was statistically significant difference in the effect on improvement （χ2 =1 838 .04 ,P〈0 .01） .The accuracy of hand hygiene of all the professional staff showed an upward trend and was increased significantly except for the accuracy of hand hygiene of the technical staff （P〈0 .01） ,and the accuracy of hand hygiene maintained more than 90 .00% till the phase Ⅲ .CONCLUSIONS The comprehensive appraisal of clinical department can remarkably raise the accuracy of hand hygiene and maintains it at high level ,and it is an effective way to improve the accuracy of hand hygiene .
