无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的疗效观察

摘　　要：目的对无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床治疗效果进行探讨和分析。方法将我院2009年1月~2012年1月行无创正压通气治疗的慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者32例设为观察组,同时选择同期行治疗的慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者26例设为对照组。比较和分析两组患者血气分析各项观察指标的治疗前后的变化情况及不良反应情况。结果观察组治疗后的p H、PO2、PCO2、HR、RR均较对照组有明显改善,差异有统计学意义（P〈0.05）;观察组治疗后无效6例,占18.75%（6/32）,明显少于对照组无效例数（12/26,占46.15%）,观察组的总有效率达81.25%,与对照组的53.85%比较存在显著性差异（P〈0.01）。结论应用无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭疗效确切,具有广泛的临床应用价值。Objective To explore and analyze the clinical effect of noninvasive ventilation in the treatment of chronic obstructive pulmonary disease combine type ]I respiratory failure. Methods From January 2009 to January 2012,the non-invasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease with type U respi- ratory failure of 32 patients was established as the observation group, selected the concomitant treatment of chronic ob- structive pulmonary disease combined type Ⅱ respiratory failure of 26 patients established as a control group. Compared and analyzed the blood gas analysis changes before and after treatment and adverse occurrence. Results The pH, PO2, PCO2, HR, RR of the observation group after treatment compared with the control group improved significantly, the dif- ference was statistically significant between groups（P〈0.05 ）. After treatment, 6 cases were invalid, accounting for 18.75% （6/32）, were significantly less than the control group （12/26, accounting for 46.15%）,the total effective rate of the observation group was 81.25%, compared with the control group with significant differences （P〈0.01）. Conclusion The application of non-invasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure is exact, with a wide range of clinical value.
