早期使用梯度压力袜对预防颅脑手术后下肢深静脉血栓形成的作用

摘　　要：目的探讨早期使用梯度压力袜在预防颅脑手术后下肢深静脉血栓(deep venous thrombosis,DVT)形成的作用。方法将186例颅脑手术后患者随机分为试验组和对照组,各93例。试验组除给予颅脑手术后常规护理外,于术后6h使用梯度压力袜;对照组只给予颅脑手术后常规护理。两组均于术后第4天常规行双下肢深静脉彩超,观察下肢DVT发生情况。结果试验组2例患者发生了下肢DVT,发生率为2.1%,对照组9例发生了下肢DVT,发生率为9.7%;试验组下肢DVT的发生率明显低于对照组,差异有统计学意义(P<0.05)。试验组下肢DVT的发生率明显低于对照组,差异有统计学意义(P<0.05)。结论早期使用梯度压力袜能提高下肢静脉血流速度,有效预防颅脑手术后患者下肢DVT的形成。Objective To investigate the role of early application of Graduated Elastic Compression Stockings in preventing deep venous thrombosis after eraniotomy. Methods A total of 186 patients performed craniotomy were randomly divided into 2 treatment groups： Experimental group （n= 93） and control group （n=93）. Patients in the experimental group were given routine nursing care associated with additional application of Graduated Elastic Compression Stockings after 6 hours of the surgery, while the patients in control group were only given routine nursing care. Four days after the surgery, all the patients were subjected to the deep venous ultrasonography,and the incidence of deep venous thrombosis in the lower extremities was noticed. Results In the experimental group, 2 patients suffered from deep venous thrombosis （2. 1%） and 14 patients suffered from calf muscular venous thrombosis （15 % ）. In control group,deep venous thrombosis and calf muscular venous thrombosis were observed in 9 （9.7%） and 26 （27%） patients respectively. The incidence of the patients in the experimental group was lower than the control group （P〈0.05）. Conclusion Early application of Graduated Elastic Compression Stockings can increase the venous flow velocity and prevent the deep venous thrombosis after craniotomy effectively.
