摘  要:目的探讨不同ST段形态早复极人群心率变异性(HRV)、心率减速力(DC)及心室复极离散度。方法研究对象早复极个体100例,分为ST段水平上升或下斜型下降50例和ST段上升或上斜型上升50例。正常对照组50例。三组患者均行24 h动态心电图检测,分析HRV、DC、心室复极离散度等指标。结果 ST段水平上升或下斜型下降组患者心率变异性明显降低,心率减速力下降,心室复极离散度增加。结论 ST段水平上升或下斜型下降的早复极个体发生恶性心律失常风险大,心率变异性、心率减速力及心室复极离散度等可以作为相关评价指标。Objective To study the heart rate variability(HRV) , deceleration capacity of heart rate (DC) and transmu- ral dispersion of repolarization(TDR) in individuals with early repolarization of different ST~segment. Methods One hun- dred cases of early repolarization(ER) were divided into horizontal/descending ST segments after ER (n = 50 ) and rapidly ascending/upsloping ST segments after ER ( n= 50). Control group contains 50 healthy individuals. Twenty-four hours dy- namic electrocardiogram inspection was performed in all research objects, and HRV, deceleration capacity of heart rate and transmural dispersion of repolarization were analyzed. Results The heart rate variability and DC in subjects with horizon- tal/descending ST variant were significantly lower compared with subjects with rapidly ascending/upsloping ST variant, and dispersion of ventricular repolarization degree were increased. Conclusion Subjects with ER and horizontal/descending ST variant has an increased hazard ratio of arrhythmic death. Heart rate variability and deceleration capacity of heart rate and transmural dispersion of repolarization have a certain clinical value on malignant arrhythmia risk assessment in early repolarization individuals.
