摘 要:目的探讨一天正常活动对健康青年志愿者的腰椎间盘平均表观扩散系数(ADC)值测量的影响。方法30例青年志愿者(男14例,女16例),年龄22~31岁。每位志愿者分别在晨起后和晚上结束一天正常活动时各进行一次MRI检查。扫描序列包括常规矢状位T1WI、T2WI及矢状位DWI。排除变性、突出的腰椎间盘,测定正常腰椎间盘早晚的ADC值,分析一天正常活动后ADC值的变化规律以及不同解剖位置(L1~S1不同水平)ADC值的变化差异。结果 30例150个椎间盘,146个纳入研究,4个椎间盘变性并突出未纳入研究。146个椎间盘早晚的ADC值分别为(1.94±0.14)×10^-3mm^2/s、(1.87±0.14)×10^-3mm^2/s,晚上较早上椎间盘平均ADC值减低,差异有统计学意义(P=0.000)。不同解剖层面,L1/2椎间盘早晚ADC值差异无统计学意义(P=0.083),L2~S1各解剖层面早晚ADC值差异均有统计学意义(t值分别为2.702、6.163、3.338、2.747,P值均〈0.05),但不同解剖位置之间,椎间盘ADC值早晚变化程度的差异无统计学意义(F值=0.985,P=0.418)。结论一天正常活动后,青年志愿者的正常腰椎间盘ADC值下降、椎间盘扩散能力减低,且解剖层面对椎间盘ADC值早晚变化的影响不明显。Objective To measure MRI apparent diffusion coefficient(ADC) value of healthy lumbar intervertebral discs in young volunteers in the morning and in the evening in order to clarify the effect of daily activities on ADC values.Methods A total of 30 young volunteers(22- 31 years old), including 14 males and 16 females,were enrolled in this study.MR T1 WI,MR T2 WI and MR diffusion weighted imaging(DWI) examinations were performed in the morning after getting up as well as in the evening after daily activity.The degenerative and protruded lumbar intervertebral discs,the ADC values of the normal lumbar intervertebral discs were determined.The results were statistically analyzed.Results A total of 146 lumbar intervertebral discs were enrolled in the study,as 4 degenerative discs were excluded.The ADC values of the normal lumbar intervertebral discs measured in the morning after getting up and in the evening after daily activity were [(1.94 ± 0.14) × 10^- 3mm^2/s]and [(1.87 ± 0.14) × 10^- 3mm^2/s]respectively.The value measured in the morning was statistically significant lower than the value measured in the evening(P 0.01).No significant difference in ADC values of L1- L2 existed between morning measurement and evening measurement(P=0.083),while significant difference in ADC values of L2- S1 existed between morning measurement and evening measurement(t=2.702,t=6.163,t =3.338 and t=2.747 respectively,P 0.05).The differences in the morning-evening change degree of ADC between different intervertebral levels were not statistically significant(F=0.985,P=0.418).Conclusion After a normal daily activities,the ADC values of normal lumbar intervertebral discs decrease and the diffusion ability of the discs will decrease.Different anatomic level has no significant impact on the change degree of ADC value between morning and evening.
