CT多平面重建及增强扫描对脐尿管癌的诊断

摘　　要：目的探讨CT多平面重建及增强扫描对脐尿管癌的诊断价值。材料与方法回顾性分析5例脐尿管癌的CT表现,包括肿瘤的部位、大小、形状及增强后的影像学表现。结果多轴位图像显示5例肿瘤均表现为中线或略偏向一侧、脐与膀胱顶部及前壁之间的占位性病变,呈类圆形1例,分叶状4例。肿块呈实性或囊实性,表现为实性2例,囊实性3例,增强后实性部分及囊壁表现为轻-中度强化、延迟强化,囊性部分无明显强化;肿瘤内或周边区域伴钙化3例。结论脐尿管癌的多层螺旋CT表现有一定特征性,CT多平面重建及增强扫描有利于脐尿管癌的定位和定性诊断。Objective To explore the value of enhanced CT and multiplanar reconstruction in the diagnosis of urachal carcinoma. Methods CT findings of 5 patients with urachal carcinoma were analyzed retrospectively, including the location, size, shape and the enhanced CT features of the mass. Results Multiplanar reconstruction images showed that tumors of all the 5 cases were located at the midline or slightly to the side, between the navel and the top and the anterior wall of the bladder. 1 case showed as oval and 4 cases as phyllodes. Two of them were solid and 3 of them were cystic and solid. Mild to moderate and delayed enhancement was showed in contrast images in the solid component in 5 cases, without any enhancement in the cystic component. 3 cases can been seen calcification in the tumor or the surrounding area. Conclusions Multi-slice spiral CT manifestations of urachal carcinoma have some characteristics in certain extent. CT Multi-planar reconstruction and enhanced CT scanning are helpful to its qualitative and positioning diagnosis.
