摘  要:目的:应用320排容积CT,分析静息态下正常人左心室各节段是否存在灌注差异。方法:对113例[男68,女45,平均年龄(54±10)岁]冠状动脉CT血管成像(CTA)显示无狭窄的CTA数据进行心肌灌注分析,采用16节段标准分段方法(排除心尖),记录各节段平均密度值(AD)和透壁灌注指数(TPR),并进行同层面或不同层面间的比较。结果:在基底段、中间段、心尖段中,室间隔壁AD和TPR分别为(88.09±16.67)Hu和1.21±0.20,(89.61±15.36)Hu和1.13±0.14,(85.34±17.09)Hu和1.24±0.17,高于同层面其余节段的平均值(P<0.001);在基底段和中间段中,前室间隔壁(2、8)AD分别为(92.28±16.71)Hu、(93.69±15.30)Hu,高于同层面其余各节段(P<0.001);心尖段AD为(77.59±18.14)Hu,低于其他两个层面(P<0.001)。结论:对于冠状动脉正常的左心室,各节段的灌注存在差异,心肌室间隔的AD和TPR明显高于其余部分,而心尖段AD相对较低。Objective:To analyze the myocardial perfusion differences among each segment of the left ventricle using 320-slice volume CT in normal adults at resting state. MethodOne hundred and thirteen cases (68 males, 45 females;mean age 54±10 years) with no stenosis in coronary arteries based on coronary CT an-giography (CTA) were included in the study. The data of CTA were used for the myocardial perfusion analysis with 16-segment standard model (excluding apex). The attenuation density (AD) and transmural perfusion ratio (TPR) of each segment were respectively recorded, and were compared in the same slice or among different slices. ResultIn basal, middle and apical slices, the AD and TPR of the ventricular septum were (88.09±16.67) Hu and 1.21±0.20, (89.61±15.36) Hu and 1.13±0.14, (85.34±17.09) Hu and 1.24±0.17, respectively, which were higher compared with those of the non-septal walls (P〈0.001). In basal and middle slices, AD of the anteroseptal myocardium (2, 8) were (92.28±16.71) Hu, (93.69±15.30) Hu, respectively, higher than those of each remaining segment (P〈0.001). The apical slice (77.59±18.14) Hu showed lower AD than basal and middle slices (P〈0.001). ConclusionIn adults with normal coronary arteries, the myocardial perfusion among each segment of left ven-tricle is different. The septum shows signiifcantly higher AD and TPR than the anterior, lateral and inferior walls. The apical attenuation shows relatively lower value.
