摘  要:目的建立同时测定人血浆中左亚叶酸及其代谢物5-甲基四氢叶酸浓度的液相色谱-串联质谱(LC-MS/MS)方法,比较并评价注射用左亚叶酸钠与注射用左亚叶酸钙在健康中国人体内的药动学特征。方法 20例男性健康受试者采用双周期、随机、自身交叉试验设计,两周期分别静脉滴注左亚叶酸钠和左亚叶酸钙100 mg。采用LC-MS/MS法同时测定左亚叶酸和5-甲基四氢叶酸的经时血药浓度,计算两种制剂的药动学参数。结果健康受试者静脉滴注左亚叶酸钠和左亚叶酸钙后,左亚叶酸的ρmax分别为(5.68±0.97)和(5.00±1.10)μg·m L-1,tmax分别为(1.01±0.03)和(1.02±0.04)h,AUC0-9 h分别为(7.45±1.11)和(6.10±1.06)μg·h·m L-1,t1/2分别为(1.1±0.2)和(1.0±0.6)h;5-甲基四氢叶酸ρmax分别为(2.18±0.48)和(1.93±0.46)μg·m L-1,tmax分别为(2.98±0.83)和(2.55±0.69)h,AUC0-24 h分别为(16.88±3.74)和(14.72±3.65)μg·h·m L-1,t1/2分别为(4.2±0.9)和(4.2±1.0)h。结论建立的LC-MS/MS法快速、准确、简便,两制剂药动学参数无显著差异,药动学特征一致。AIM To establish a sensitive and simple liquid chromatography tandem mass spectrometry( LC- MS/MS) method for simultaneous determination of levofolinic acid and its metabolite, 5- methyltetrahy-drofolate in human plasma, and to compare and evaluate the pharmacokinetic characteristics of disodium levofolinate and calcium levofolinate for injection in Chinese healthy volunteers. METHODS Twenty male healthy volunteers received an intravenous infusion of a single 100 mg dose of disodium levofolinate and calcium levofolinate in a randomized two- way crossover design and the plasma concentration of the drug was assayed by LC- MS/MS. The pharmacokinetic parameters of the two formulations were calculated. RESULTS The major pharmacokinetic parameters of disodium levofolinate and calcium levofolinate were as follows: ρmaxwere(5.68 ±0.97) and(5.00 ± 1.10) μg·m L-1, tmaxwere(1.01 ± 0.03) and(1.02 ± 0.04) h, AUC0-9 hwere(7.45 ± 1.11)and( 6.10 ± 1.06) μg·h·m L-1, t1/2were( 1.1 ± 0.2) and( 1.0 ± 0.6) h, respectively. The major pharmacokinetic parameters of the metabolite were as follows: ρmaxwere(2.18 ± 0.48) and(1.93 ± 0.46) μg·m L-1, tmaxwere(2.98 ± 0.83) and(2.55 ± 0.69) h, AUC0-24 hwere(16.88 ± 3.74) and(14.72 ± 3.65) μg·h·m L-1, t1/2were( 4.2 ± 0.9) and( 4.2 ± 1.0) h, respectively. CONCLUSION The LC- MS/MS method is convenient and accurate. There are no significant differences in pharmacokinetic characteristics of disodium levofolinate and calcium levofolinate for injection.
