高龄患者行完全腹腔镜肝癌切除术1例暨文献复习

摘　　要：目的:探讨高龄肝癌患者行完全腹腔镜下肝癌切除术的围手术期管理和治疗。方法:2014年4月1日我院完成1例84岁高龄患者完全腹腔镜下肝癌切除术,并复习相关文献。结果:该高龄患者于全麻下行完全腹腔镜下肝癌切除术,手术顺利,手术时间90 min,术中出血200 mL,无胆漏及术后出血,肝功能恢复快,术后第9天出院。结论:完全腹腔镜下肝癌切除术适用于高龄患者,具有手术创伤小,术后恢复快,并发症少等特点。该技术可进行临床推广,为高龄肝癌患者提供新的手术治疗途径。Objective： To investigate the perioperative management and treatment of elderly liver cancer patients undergoing laparoscopic liver resection totally. Methods： On April 1, 2014, an elderly patient underwent totally laparoscopic liver resection in our hospital, and the literatures concerned were reviewed. Results： The laparoscopic liver resection in the elderly patient was completed successfully under general anesthesia. The operation time was 90 min, and the intraoperative blood loss was 200 mL. There were no bile leakage and postoperative bleeding. Moreover the liver function of the patient recovered quickly and postoperative hospitalization was 9 days. Conclusions： Totally laparoscopic liver resection is suitable for elderly liver cancer patients, with some advantage characteristics, such as small surgical wound, fast recovery, fewer complications,etc. The technology could be promoted to clinical application, which offers a new approach for elderly patients with liver cancer.
