摘 要:目的:评价网络互动式健康教育对糖尿病患者自我管理水平的影响。方法选择2010年9月-2012年3月在某院内分泌科住院的糖尿病( DM)患者140例,采用随机数字表法随机分为观察组和对照组各70例。对照组通过健康教育讲座进行患者教育;观察组除参加健康教育讲座外,每周采取网上互动方式,讲解和答疑患者遇到的问题。比较两组患者的自我管理水平、血糖控制情况及生活质量。结果出院6个月,观察组患者自我管理总分(89.35±15.21)分、运动依从性(77.42±17.15)分、药物依从性(98.73±15.81)分、足部护理(81.24±21.94)分,均高于对照组(80.23±12.37),(69.32±17.35),(92.31±16.78),(72.59±22.73)分,差异有统计学意义(t 值分别为3.892,2.778,2.014,13.301;P<0.05);观察组患者餐后2 h血糖2 h PPG(6.43±1.40)mmol/L,糖化血红蛋白HbA1c(7.02±1.10)%,均低于对照组(8.06±0.91)mmol/L,(8.29±1.04)%,差异有统计学意义(t值分别为8.167,7.019;P<0.01);观察组SF-36问卷中体能影响维度(57.2±21.7)分,一般健康(63.1±16.4)分,精神影响(67.1±23.5)分,心理健康(83.3±9.1)分,均高于对照组(49.7±23.2),(52.4±17.8),(59.9±26.4),(69.2±13.7)分,差异有统计学意义(t值分别为2.417,3.147,2.559,3.527;P<0.05)。结论网络互动式健康教育在患者出院后,效果优于健康教育讲座,有利于巩固住院的健康教育效果。Objective To evaluate the effects of interaction network health education on the self-management in patients with diabetes mellitus .Methods A total of 140 diabetes patients were selected in our hospital from September 2010 to March 2012 , and were divided into observation group and control group through a random number table .Patients of control group received health education seminars .Except to education seminars, the patients in the observation group received real-time network interaction education once a week , with a total of 24 times.And the patients of two groups were assessed with the level of self-management , glycemic control level and the quality of life .Results Six months after discharge , the self-management score , the compliance of exercise , the compliance of medication and foot care were ( 89 .35 ±15 .21 ) , ( 77 .42 ± 17.15),(98.73 ±15.81) and (81.24 ±21.94) in the observation group, which were significantly higher than the control group with (80.23 ±12.37),(69.32 ±17.35),(92.31 ±16.78) and (72.59 ±16.78) (t=3.892,2.77,2.014,13.301, respectively;P 〈0.05).2 hour postprandial blood glucose and glycosylated hemoglobin level in the observation group were (6.43 ±1.40) mmol/L and (7.02 ±1.10)%, which were significantly lower than the control group with (8.06 ±0.91)mmol/L and (8.29 ±1.04)%(t=8.167,7.019, respectively;P〈0.01).The scores of physical dimensions , general health, spiritual impact and mental health of the SF-36 questionnairein the observation group were (57.2 ±21.7),(63.1 ±16.4),(67.1 ±23.5) and (83.3 ±9.1), respectively, which were significantly higher than the control group with (49.7 ±23.2), (52.4 ±17.8),(59.9 ±26.4) and (69.2 ±13.7) (t =2.417,3.147,2.559,3.527, respectively;P 〈0.05).Conclusions Interactive network of health education is better than traditional health education seminars, and can help consolidate the effects of inpatient health education .
