摘  要:目的探讨妊娠期膳食因素在妊娠期高血压疾病(HDP)发生中的作用,为制订有效的膳食营养干预措施提供科学依据。方法采用1∶1匹配的病例对照研究方法,对204例HDP孕妇(病例组)和同期同所医院正常妊娠的204例孕妇(对照组)进行食物摄入种类和频率的问卷调查,比较两组孕妇妊娠期食物摄入的频率,并采用单因素分析和多因素条件logistic回归分析,筛选出与HDP发生相关的膳食因素。结果病例组妊娠期摄入粮谷类和蔬菜水果类与对照组相比,差异无统计学意义(P>0.05);病例组妊娠期摄入肉禽鱼类、蛋类、奶类、豆类、干果类的频率低于对照组,摄入咸菜泡菜类的频率高于对照组,差异有统计学意义(P<0.05)。单因素logistic回归分析初步筛选出6项相关因素。多因素条件logistic回归分析显示,妊娠期摄入蛋类(OR=0.770)、奶类(OR=0.809)是HDP的保护因素;摄入咸菜泡菜(OR=1.17)可能是HDP发生的危险因素。结论妊娠期多食蛋类、奶类可有效预防或减少HDP的发生,而摄入咸菜泡菜会增加孕妇发生HDP的风险。Objective To investigate the impact of diet on hypertensive disorders in pregnancy(HDP),and to pro- vide reference for diet intervention. Methods A case-control study was conducted with 204 pregnant women with HDP and 204 normal pregnant women. The types and frequency of various foods intake during pregnancy were investigated by questionnaires. The impact of diet on HDP was analyzed using logistic regression analysis. Results The frequency of grains and vegetables intake were comparable between two groups(P〉0.05);the frequencies of fish, meat,eggs,milk,beans and nuts intake in the case group were significantly lower than those of the control group (P〈0.05);while the frequency of pickles intake in the case group was significantly higher than that of the control group(P〈0.05). Logistic regression analysis showed that intake of eggs(OR=0.770) and milk(OR=0.809) were the protective factors of HDP,while intake of pickles(OR=l.17) was the risk factor of HDP. Conclusion Intake of eggs and milk can effectively prevent or reduce the incidence of HDP,while intake of pickles would increase the risk of HDP.
