曲靖市2009～2013年吸毒人群艾滋病哨点监测分析

摘　　要：目的分析曲靖市吸毒人群艾滋病性病流行趋势及相关行为变化情况。方法2009～2013年，对曲靖市强制戒毒所每年新人所的全部吸毒者开展行为学问卷调查，并进行HIV、丙型肝炎和梅毒抗体检测。结果共监测2148人，艾滋病知晓率（72．2％～96．1％）逐年上升（X2=86．747，P〈0．01）；HIV阳性率（12．3％～6．7％）逐年下降（X2=9．574，P=0．048）；HCV阳性率（14．5～38．3％）逐年上升（P〈0．01）；梅毒阳性率（0．6～2．5％）、安全套使用率（19．5％～78．8％）、共用过针具率（5．1％～40．0％）和最近1次共用针具率（1．2％～10．9％）的各年度差异均有统计学意义（X2=81．806，X2=177．346，X2=313．327，X2=166．180，P〈0．01）。安全套发放，咨询检测接受率、社区药物维持治疗腈洁针干预接受率和同伴教育接受率分别为41．9％～100．0％、35．8％～83．0％、22．4％～92．4％。结论吸毒人群HIV阳性率逐年下降，但HCV阳性率逐年上升，梅毒感染率仍然很高。安全套使用率、共用针具率及干预服务接受率波动较大。Objective To undersand the prevalent trend of AIDS and STD and the changes of behavior among drug user in Qujing city. Methods The behaviors of drug users in drug rehabilitation center in Qujing city were surveyed with questionaire and antibodies against HIV, hepatitis C virus and syphilis during 2009 to 2013 were tested. Results A total of 2 148 drug users were surveyed. The AIDS awareness rate （72.2%-96.1%）increased year by year （X2=86.747,19 〈 0.01）. The HIV positive rate （12.3%-6.7%） reduced year by year （X2=9.574, P=0.048）.The HCV positive rate（14.5-38.3% ）increased year by year（X2=81.806, P 〈 0.01）.The syphilis positive rate （0.6-2.5%）, the condom using rate （19.5%-78.8%）, the shared needle rate （5.1%-40%） and shared needle rate in the last sex （1.2%-10.9%） were statistically significant in different years（X== 81.806, X2=177.346, X2=313.327, X2=166.180, P 〈 0.01）.The rates of condom/counseling and testing acceptance, community drug maintenance treatment/clean needle intervention and peer education 41.9%-100.0%, 35.8%-83.0%, 22.4%-92.4%, respectively were .Conclusion The HIV positive rate reduced year by year among drug user, but the HCV positive rate increased year by year, and syphilis infection rate has been very high . Meanwhile, the condom using rate, the shared needle rate of and intervention service acceptance rate fluctuated greatly.
