MPB64抗原检测对非结核分枝杆菌快速鉴别的意义

摘　　要：目的探讨结核分枝杆菌MPB64抗原在快速鉴别非结核分枝杆菌的诊断中的应用价值。方法对89例痰结核分枝杆菌培养阳性标本,进行培养液结核分枝杆菌MPB64抗原免疫胶体金法检测,并对结果进行比较分析。结果25例确诊非结核性标本,抗原检测均阴性;64例确诊结核性标本中,62例抗原检测阳性,阳性率为96.9%。用MPB64抗原胶体金法检测非结核分枝杆菌,敏感性为96.9%,特异性为100%,诊断一致率为97.8%。结论分枝杆菌MPB64抗原胶体金法可快速鉴别结核分枝杆菌及非结核分枝杆菌,具有高度的特异性和敏感性,是早期诊断与鉴别诊断非结核分枝杆菌感染的一种有价值的检测方法。Objective To analyze the value of MPB64 immunity conoid gold in rapid identification of nontubereulous myeobaeteria. Methods The MPB64 immunity colloid gold was used to detect the MPB64 antigen in 89 cases positive for Mycoeobacterium tuberculosis including 25 nontubereulous myeobacteria cases and 64 tuberculous cases by sputum culture. The results were statistically analyzed. Reaults The results of detection of 25 eases of nontuberculous myeobacteria with MPB64 immunity colloid gold method were all negative , and 62 of the 64 tuberculosis cases were confirmed positive for Myeobacterium tuberculosis with a positive rate of 96.8%. The sensitivity, specificity, and accuracy of MPB64 immunity colloid gold method for detection of nontubcreulous myeobacteria were 96.9% （62/64）,100% （25/25）,97.8% （87/89）, respectively. Conclusion Mycobacterium tuberculosis MPB64 immunity colloid gold is a simple, rapid and highly sensitive test for identifying patients with nontuberculous mycobacteria from those infected with Myeobacterium tuberculosis.
